NURSING DIAGNOSIS Sleep Deprivation related to sustained environmental stimulation; uncomfortable environment

INFERERENCE Exposed to multiple stressors (loud portable dvd sound, restricted movement)

GOALS AND OBJECTIVES SHORT TERM GOAL After 2 hours of nursing intervention, the client will be able to report 1.Verbalized the causative/contributi ng factors of his sleep deprivation 2.Know the importance of enough sleep in our body 3.Understand proper sleep inducing techniques LONG TERM GOAL

NURSING INTERVENTION *Determine presence of physical or psychological stressors, including recent injury *Note environmental factors affecting sleep *Determine clients usual patterns and expectations *Ascertain duration of current problem and effect on life/functional ability *Observe for physical signs of fatigue (e.g. restlessness) *Enumerate what are the benefits of sleep in our health

RATIONALE Provide comparative baseline

EVALUATION After 8 hours of nursing intervention, the client was able to report improvement in sleep pattern as manifested by: verbalizing the causative/ contributing factors of sleep deprivation and knowing importance of enough sleep GOAL IS MET

Compromised sleeping pattern

Determine the underlying cause of his problem Helps identify appropriate options Helps client understand the the importance of achieving enough sleep A chronic sleeprestricted state can cause fatigue To decrease tension and prepare for rest/sleep.To discuss the importance of enough sleep

Loss of normal sleeping hours Prolonged periods of time without sleep

Deprive a person of vital requirements of the body

After 8 hours of nursing intervention, the client will be able to 1. Establish optimal sleep pattern

*Encourage client to develop plan to restrict caffeine, alcohol, and other stimulating substances from late/evening intake. Instead he or she may drink milk if not contraindicated *Recommend quiet activities such as reading/listening to soothing music in the evening. Recommend the need for calm, quiet environment and manage controllable sleepdisrupting factors (e.g. noise, light, room temperature) I

These factors are known to disrupt sleep pattern. Caffeine significantly affect visual vigilance, choice reaction time, repeated acquisition, self-reported fatigue and sleepiness. These will reduce stimulation so the client can relax

To enhance client's ability to fall asleep
