Hazards Cause of shutdown in the plant area.The work force doesnot understand the pre task briefing therefore does not understand the potential dangers and control measures to be implemented. Some of the workers could be lacking PPE or using defective PPE.

Prior to mobilization; conduct the Safety tool box talk to the workers to be aware Imco/Client on the possible cause of hazards in the working area. X-Ray

Prior to start x-ray work, a hot work permit must be submitted to gain client end user approval. X-ray activity must be carried out in the fixed area. . .(..) X-ray devices must be approved by Client or end user before use. The x-ray activity must be carried out during the night or other restricted working hours as agreed. Alarm signal/flashing lights must be installed including barricading of exposure areas.

All ways uses high pressure with proper connection and wipe check should be used. Clear of hands and figures from getting trapped in between. Make sure, gauges / recording devices & test accessories are tested, calibrated and having valid calibration certificates. Follow approved test procedure and use trained personnel to operate and monitor the activity.
