Please select teachers who have taught you during the 11th or 12th grades and have awarded you a grade that appears on your transcript
Teacher evaluations lrom counselors, principals, or coaches will only be accepted il they meet the alorementioned criteria
Complete the student section lor each teacher
High school students-give both pages ol the teacher evaluation to TWO teachers lrom dillerent subject areas, i.e., English and science,
math and music
Transler students-give both pages ol the teacher evaluation to one high school teacher and one college prolessor
ll you attended college lor one year or more, give both pages ol the teacher evaluation to two college prolessors

Under the lederal law entitled the Family Educational Pights and Privacy Act ol 19?4 and according to the University ol Oregon Student
Pecord Policy; registered students are given the right to inspect their records, including letters ol recommendation and teacher evaluations.
Opinion is divided whether letters open to review are more helplul in assessing a student's potential than those that are not. ll the student
wants to waive their right ol access to this evaluation, they may do so by signing the waiver below. ln any case, all evaluations will be carelully
considered.

The Pobert D. Clark Honors College is a lour-year liberal arts college within the University ol Oregon and includes students in all scholarly
