EPITENS TAB.

Composition
Each tablet contains:
Xipamide 10 mg
Triamterene 30 mg

Properties and Mode of Action


Xipamide is a thiazide-like diuretic structurally-related to indapamide,acting on cortical thick ascending limb of the Loop of
Henle and the early part of the distal tubule interfering with reabsorption of sodium chloride and potassium at these sites
increasing their excretion.
Triamterene is a potassium-sparing diuretic which acts directly on the late part of the distal tubule and collecting duct
interfering with sodium reabsorption at these sites,thus decreasing potassium secretion.
This combination eliminates excessive salt and water.
Consequently, it reduces the blood volume and pre-load leading to lowering of elevated blood pressure and also control of
heart failure without altering serum potassium level.
EPITENS also reduces the sodium content of vascular smooth muscle cells, thus decreasing the vascular tone resulting in
reduction of the peripheral resistance and consequently, lowering of the elevated blood pressure (additive antihypertesive
effect).

Indications
Mild to moderate hypertension.
All kinds of oedema in tissues or body cavities due to heart failure, kidney, liver or venous diseases.
In combination with cardiac glycosides for the treatment of oedema and high blood pressure.
Basic therapy of severe hypertension in combination with other antihypertensives.

Dosage and Administration


Hypertension:
Unless otherwise prescribed:
1 tablet daily, preferably after breakfast.
Oedema and Heart Failure:
1 - 2 tablets daily or every other day according to the severity of the disease. If necessary, the dose may be increased to
3 tablets daily for a short period.

Precautions
Hepatic coma.
Sulphonamide hypersensitivity.
Pregnancy and lactation.

Drug Interactions
The antihypertensive effect of EPITENS can be augmented by other blood pressure-lowering drugs, barbiturates,

phenothiazine, tricyclic antidepressants as well as vasodilating agents and by alcohol intake.


The effects of antidiabetics, uricosuric drugs, noradrenaline and adrenaline may be attenuated.
Simultaneous administration of spironolactone or potassium-containing drugs with EPITENS should be avoided as this may
result in elevated potassium level in patients with impaired liver and/or kidney functions.

Contra_indications
EPITENS is not recommended in renal patients with creatinine clearance of less than 10 ml/minute because of its
triamterine content.

Side effects
EPITENS is well-tolerated.
Side effects are rather rare; including epigastric pain, limb pain, dry mouth, headache and vertigo.
These side effects usually disappear after a few days of treatment.

Presentation
3 blisters of 10 tablets each.
