(2) The CMA will annotate the medical record entry with
a statement indicating that the individual and or individual's
records have been screened under the reliability standards of
this instruction. If PDI was identified, the CMA will provide
an evaluation and recommendation to the certifying official in
sufficient detail so he or she can make a sound decision
concerning the individual’s suitability for the BPRP. The
medical record entry will indicate the nature of the PDI and the
date the certifying official was notified. The medical record
entry will include the name, grade, and signature of the CMA and
the date of the screening.
