RESUMO
INTRODUÇÃO
OBJETIVO
Demonstrar a experiência do Serviço de Oncologia Pediátrica do Centro
de Cirurgia Pediátrica no manejo das neutropenias febris observadas entre
1996 e 2002.
MATERIAL E MÉTODOS
Estudo retrospectivo dos pacientes pediátricos com tumores sólidos
malignos, tratados com quimioterapia, de janeiro de 1996 a junho de 2002.
Foi considerada como neutropenia, a contagem de neutrófilos menor que
1000/mm3 e, como febre, temperatura axilar maior que 37,7oC. Foram
obtidos dados dos prontuários em relação ao diagnóstico, episódios de
neutropenia febril, resultados das culturas, uso de fator estimulador de
colônias de granulócitos e terapêutica.
RESULTADOS
De 45 crianças tratadas com quimioterapia, 22 apresentaram episódios
de neutropenia febril, totalizando 46 episódios. A mediana da contagem de
neutrófilos foi 440/mm3 (amplitude: 230 a 1.050). Dos 46 episódios, em 30
(65%) foi colhido material para hemocultura, pelo menos uma vez, com
positividade de 53%. Foi colhida urina para cultura em 32 (69%), com oito
positivas (25%). A coleta de secreções foi feita em seis casos, com

CONCLUSÃO
Como previsto, a neutropenia febril foi mais freqüente naqueles
pacientes que necessitaram de esquemas quimioterápicos mais
agressivos, causando depressão medular intensa. Esses casos foram
tratados, em sua maioria, com utilização de ceftriaxona e amicacina,
associadas à filgrastima. O percentual de coleta de culturas foi menor
que o esperado. Da mesma forma, poderia esperar-se que o índice de
positividade das hemoculturas e das uroculturas fosse maior. Entre-
tanto, como observado também em outros estudos, nos casos de
neutropenia febril, pode ser difícil isolar o agente patogênico.
