COPE OF THE EXAMINATIONS

ASSING GRADE
o pass the test, the examinees should get a rating of at least 80.00.
DMISSION REQUIREMENTS
pplicants are advised to check carefully whether they meet the admission requirements stated below.  Only those who meet the
dmission requirements should apply for the examination, otherwise, their applications shall be disapproved and the examination f
aid shall be forfeited.
At least 18 years of age on the date of filing of application;
- for May 22, 2011 CSE-PPT, applicant must be born on or before February 21, 1993
- for October 16, 2011 CSE-PPT, applicant must be born on or before May 23, 1993
Must be Filipino citizen;
Must be of good moral character;
Have not been convicted by final judgment of an offense or crime involving moral turpitude, or disgraceful or immoral condu
dishonesty, examination irregularity, drunkenness or addiction to drugs;
Have not been dishonorably discharged from military service or dismissed for cause from any civilian positions in the govern
Applicant with pending administrative and/or criminal cases may take the examination and shall be conferred the eligibility if
successfully pass the same.  However, this is without prejudice to the outcome of their pending cases.  If they are found guilty
grave offenses, their eligibility shall be forfeited based on the penalties stated in the decision and pursuant to Section 58 (a) of
Uniform Rules on Administrative Cases in the  Civil  Service and CSC Resolution No. 010659, dated March 22, 2001; and
Have not taken the same level of CS Examination in less than three (3) months.

PPLICATION REQUIREMENTS
Applicants must accomplish properly the prescribed examination application form (CS Form No. 100, Revised 2008).
Four (4) copies of identical I.D. pictures with specifications, as follows:
a. (4.5 cm x 3.5 cm - 1.78" x 1.38") Passport size
b. Colored with white background
c. Printed on quality photo paper

Must show the subject applicant in standard close-up shot (from shoulder level up).  Applicants should not wear eye glas
any accessory that may cover their facial features when they are having their pictures taken.
e. Taken within the last three months prior to filing of application
f. With full name tag
  - The name tag must show the applicant's First Name,  Middle Initial, Last Name, and Extension Name (if any)
The name tag should not be computer-generated, meaning, the applicant should have his/her picture taken while hold

his/her name tag.

   

Any valid ID such as Driver’s License, SSS ID, GSIS ID, current Office ID, Postal ID, BIR ID, Barangay ID, Voter’s ID, Val
Passport or Police Clearance which contain the applicant’s picture, signature, birthdate, and the signature of the authorized he
the issuing agency.  Photocopy of the same shall be attached to the application form to serve as reference for Room Examiner

Examination fee of Php 500.00.

Failure to take the examination on your scheduled date shall mean forfeiture of your examination fees.  No request for re-
scheduling is allowed.

WHERE TO GET AND FILE APPLICATION FORMS


pplication forms are available at any CSC Regional/Field Offices.  The same may be downloaded from the CSC website:

pplication forms may be reproduced in 8.5 in. x 13 in. (21.6 cm x 33 cm) size bond paper provided the same format is followed. 
pplicants may file their applications in any of the CSC Regional/Field Offices.

ualified applicants shall be notified of their places of examination (school assignment) through a Notice of Assignment.  If appli

ssignment.  To use this system, the examinees should log on to the CSC website and go to the Examination Announcement webp

WHAT TO BRING ON EXAMINATION DAY
pplicants should bring the following on examination day:
Notice of Assignment or Examination Permit
Lead Pencil/s (No. 1 or 2) and eraser/s
Ball Pen (Blue or Black)
The same valid ID cards presented during application stage (i.e. Driver’s License, SSS ID, GSIS ID, current Office ID, Postal
BIR ID, Barangay ID, Voter’s ID, Valid Passport or Police Clearance)

WARNING

The Civil Service Commission uses a highly reliable system to detect cheats. Any form of
cheating in any civil service examination shall be considered a violation of Republic Act
9416  and any person found guilty shall be administratively and criminally liable.

Those found ADMINISTRATIVELY LIABLE shall suffer the following penalties:  For
those in the GOVERNMENT SERVICE, dismissal from the service with all the
accessory penalties of forfeiture of government benefits, cancellation of eligibility, bar
from taking any government examination, and perpetual disqualification from re-entering
government service.  For PRIVATE INDIVIDUALS, perpetual  bar from entering
government service and from taking any government examination.

Those found CRIMINALLY LIABLE shall be meted out the penalty of imprisonment
of not less than six (6) years and one (1) day but not more than twelve (12) years and a
fine of not less than Fifty Thousand Pesos (Php50,000.00) with the appropriate accessory
penalties.
