NOTE:
The person making this report shall also fill in the blank spaces under "Report
onPromotions" on this side of the form.1. This form will be made out in duplicat
e at the close of the school year by each teacher who is in charge of the enrolm
ent and attendance recordsof any class in Grades I, II or III. The original copy
should be retained onfiel in the office of the principal of the school and the
duplicate sent to thedivision office for filiing.2. In the first column will be
written the names of all pupils who have beenenrolled during the year in the reg
ister fo the class for which this report ismade. List names of boys first.3. Und
er "Years in School"; write 1, 1-1/2, 2, 2-1/2, 3, 3-1/2, etc. to indicatethe ex
act lenght of time the pupil has been in school form the first time heentered an
y school to the date accomplishing this form.4. The age of the pupil as reported
on this form should be his age as of March 1. To find his age as of April 1, ad
d to his age as of the begging of the school year, recorded in Dep Ed Form 1- Sc
hool Register.5. Under "Home Address" Should be written the actual residence of
thepupil, not a temporary residence which he may have taken up whileattending sc
hool.6.A pupil who is promoted during the year will be reported in each grade in
which he is enrolled.7. In the column for "Final Rating" should be indicated the
final ratingreceived by the pupil during the months he was enrolled in the grad
e for which the report is made. For example, if a pupil is enrolled in Grade I i
nJuly and is promoted to Grade II in September, he should be reported onDep Ed F
orm 18-E -1 both for Grade I and Grade II. His final rating for Grade I will be
based upon his work form July to September. His finalrating for Grade II will be
based upon his work form October to March, andnot on those months in which he w
as studying in Grade I. The term "finalrating" signifies either the average of a
ll the periodic ratings according tothe averaging system of grading or the last
periodic rating, after the first, isobtained by dividing 3 the sum of twice the
tentative rating and theperiodic rating immediately preceding: and the last peri
odic ratingbecomes the final subject rating.\8. Great care should be exercised t
o make sure that the total number of days the pupil has attended the grade in bo
th present and past schoolyears is correctly recorded.9. Under "Remarks" indicat
e all pupils promoted during the school year tothe grade for which the report is
made by writing the words "Promotedfrom Grade" giving the number of the grade a
nd the date on which thepromotion ws made. Likewise indicate all pupils promoted
to a higher grade from the grade for which report is made by writing the words"
Promoted to Grade" giving the number of the grade and the date onwhich the promo
tion was made. Indicate all Grade I pupils who enter thegrade for the first time
during the school year by writing the words "Enteredschool " giving the date on
which the pupil entered.10. When the blanks under "Summary of Pupil Enrolled" a
re filled in thefollowing points should be borne in mind.(1) The march montly-en
rolment figures gives on the first lineshould agree with the monthly enrolment f
igures given on Dep Ed Form 2for March.(2) The data for the second line should i
nclude all pupils promotedto the next higher grade before the end of the school
year.(3) The date for the third line may be found by adding the figureson the fi
rst line to the figures on the second line.11.Data for the "Summary of Pupils Pr
omoted" may be obtained from theface of the form.12. The sum of the ages of all
pupils concerned should appear where'Total Age of Pupils" is called for. For exa
