This contract is entered into by the Mainely Rat Rescue (hereinafter referred to as “MRR”) and the Adopter
named above. This contract pertains to the animal listed above. Both the MRR and the Adopter recognize and
agree that this is a valid contract; enforceable in a court of law and equity, and that they intend to be bound by
it. In receiving this animal from MRR, I, the undersigned, agree to comply with the following:

Terms of Adoption:

1. I will provide the animal with a clean, spacious and safe cage using only MRR approved bedding materials, a
balanced diet, fresh water and toys for mental stimulation as well as daily handling.
2. I agree that this animal will not be used to feed any other animal or human and will not be used for breeding.
3. I will provide, at my expense, veterinary care for illness, injury and care as prescribed by a veterinarian or
specified by MRR.
4. I understand that MRR cannot guarantee the health or temperament of any animals, or be responsible for
expenses incurred as a result thereof.
5. I understand that an animal’s behavior may change when he/she enters a new home or environment and I am
willing to give the animal the time he/she needs to adjust.
6. I understand that animals should be closely supervised when they are with children.
7. I understand that MRR is a nonprofit organization, not a pet store, and that the adoption fee is used to defray
the cost of caring for all the animals and is nonrefundable.
8. In the event that I can no longer care for the animal, I will return him/her to MRR for disposition at MRR’s
discretion.
9. I will make this a household pet and companion, to live in my home and not left outdoors, kept in a barn,
shed, garage, basement or any space not usually occupied by my family. I will allow any agent of MRR to
inspect and remove the animal from my premises if the agent is not satisfied with the conditions under which it
is being kept.
10. I agree that failure to abide by the terms of this contract may result in the animal being confiscated and
returned to MRR and/or legal action.
11. I do hereby forever release MRR, its administrators, employees, officers, members, agents and their heirs
from any and all claims, damages, costs, known and unknown, of said caregiver arising out of any action or
occurrence, particularly on the account of this adoption.
