Objective of Questionnaire
DOE has introduced new registration scheme for individuals that has been implemented since the
1st of June 2007. During the Stakeholders Consultation Forum for EIA Consultants on the 9 th of
January 2007, many EIA consultants have requested DOE to also reintroduce the company
registration.
DOE has conducted the 1 st Stakeholders Consultation forum on the Criteria for Company
Registration on the 2nd of October 2007. We have received mixed res ponses on the needs for the
registration and the criteria proposed. Most of the comments we received are quite general in
nature.
Therefore in order to assist us in better decision makin g, DOE has decided to conduct this survey
to allow the us to gather more accurate information and feedback on the Criteria for Company
Registration and other related matters. Thus, the DOE would request the sincere cooperation of
consulting companies in providing the required information .
Second consultation meeting will be held on the 26 th of October 2007 at Bilik Gerakan,
13th Floor, Menara Sumber Asli , NRE at 9.00 am .

The information provided in this questionnaire will be kept

confidential. It will only be used for the sole purpose of
formulating appropriate criteria and requirements

4. The participation is opened for both companies or institution previously registered under
the old registration scheme and also unregistered companies , that has been conducting
EIA studies. (Note : Institutions refer to universities such as UTM, UPM etc)

5. All responses must be received no later than 24th of October 2007 at 12.00 pm.
