It’s the 1960s. Susanna Kaysen, 19 years old, recently took an overdose of
aspirin and vodka, although she insists it was not a suicide attempt. She has
been sent to Claymoore for an extended hospital stay (common for this time
period).
What is the chief complaint? (In the client’s words, and report of others)
Response:
Borderline Personality Disorder
Based on the above information and a close viewing of the movie, what
questions would arise during history taking? What are some possible
answers? You might base your questions on:
History of the client’s illness.
Psychosocial history.
Family history or dynamics.
Response:
How often does she feel emptiness?
Have you ever tried hurting yourself? Does hurting yourself help ease
your emptiness?
What do you do when you get angry do you hold it inside or let loose
with it so everybody knows how you're feeling?
-based on the movie she did try to commit suicide and then the presence
of bruise on the ER means that she tries to hurt herself expressing her
emptiness and anger
Response:
Sometimes she looks so confused and then when she answers the
questions she cannot answer the following questions because she is
thinking about something else. She also looks flustered all the time
What requirements does the client meet that support this diagnosis, based
on DSM-5 criteria?
Response:
The presence of bruises in her hands, Her thought of suiciding and
always depressed and later on in the movie she felt emptiness
Response:
I would recommend her to visit a psychiatrist to deal with her suicidal
thoughts and depression and take the prescription that is recommended
by the doctor.
With what expected outcomes?
Response:
She will learn how to handle depression everytime that she will
experience an episode and take her medication on time

She always skips her meds, making trouble, and manipulating her fellow
patients. She will do anything to get what she wants, and she's been
known to seduce guards to stop them from telling on her.
Based on the above information and a close viewing of the movie, what
questions would you raise during history taking? What are some possible
answers? You might base your questions on: Past psychiatric history,
treatment, treatment outcomes
Note: Past history of the illness, psychosocial history, and criminal history
would be important areas to assess with a client such as Lisa; however,
the movie doesn’t give information about Lisa’s history prior to her stay at
Claymoore.
What was her family like?What kind of environment did she grow up to?
How did she end up in the hospital?
When did it start?
How does she vent her anger? Does she hurt herself or she hurts others?
What observations do you have about the client’s behavior?
Response:
Based on the movie Lisa manipulated Daisy and Susanna for her own
gain and pleasure. She is really confident her self esteem is really high.
She doesn’t realize the wrong things she’s doing that is the reason why
she is also considered as a psychopath. She seeks attention and she
wants every patient to admire her
Response:
Yes, because of the way she is acting, she is impulsive,manipulative and
doesn’t know how to distinguish what is right and wrong
What requirements does the client meet that support this diagnosis, based
on DSM-5 criteria?
Response:
-She manipulated daisy and susanna
-she exploited daisy
-she is sleeping with the guard so that they will not tell her
-she seeks attention
-she steals money
-she is aggressive
-she doesn’t care if what she did is right or wrong as long as she gets
what she wants
Response:
I would recommend a psychiatrist, also she needs to have a teaching on
how to abide by the rules of hospital. She will take the prescribed
medications

-she will learn how to express herself in a nondestructive behavior
-she will take her medicines
