Childcare Contract

I understand the Parent Handbook explains all of the policies in place at

The Daycare including: payments, vacations and absences, termination, and
discipline. I also understand that contracts are renewed annually at which time I will
receive a new copy of the updated Parent Handbook. Should changes be
made to the Parent Handbook throughout the year, I will be notified in
writing.
This is an agreement between The Daycare and

childcare for _________________________[child] beginning
____________________[date]. This agreement is for one full year, unless agreed
upon and signed by both parties in writing.

The following documents and fees will be required before your child may be placed in
care:
 Completed Child Care Application for Enrollment
 First weeks’ tuition
 Deposit
 This Agreement signed by both parents or custodial parent
 All other forms required for your child’s specific needs (infant forms, supply list
agreements, prescription medication forms, etc.)

At the time of admission, the undersigned parent or guardian understands that care
will be billed at the rate of _____ per week based on care being provided for the

I agree that I have received a copy of the Parent Handbook for The Daycare. I have
read it thoroughly and agree to abide by the policies set forth in the guide.
