SAFETY AND HEALTH DEPARTMENT FIRST
Quality documentation is a must in the Safety. Essentially,
accurate documentation is necessary where a physical process or
activity is conveyed through the medium of writing by having it
noted down in a document of some kind.

This is a necessity due to several reasons:

• It gives us history

• Documents to review

MINIBOSH.022411.Maximo Ginez III – mxginez3@yahoo.com

• Evidence for follow up investigation

• It proves that we are working properly

• Traceability of all events

The important part of managing safety is to make sure that we

always have clear precise documentation of every single important
events, process, and action that takes place. This will allow you to
have a clear reference point whenever anything is in question.
Proper documentation shall be done and maintained regularly,
safe keeping, storage and filed according to its category.

hat is a Toolbox Meeting?
Toolbox meetings are short safety meetings covering a
specific safety topic or type of hazard.

Common topics for these meetings include one or more of the

following:

MINIBOSH.022411.Maximo Ginez III – mxginez3@yahoo.com

1. Hazards of a job that is about to be started.
2. Causes and corrective action for a recent accident or
“near miss.”
3. Job hazards that should be emphasized along with
safety controls that will prevent accidents.
4. General problems concerned with both on-job and off-
job safety.
5. Seasonal safety information

How large should a Toolbox Safety
Meting be?
The ideal size for a toolbox safety meeting is 6 to 10 workers.
Larger groups can be used, however, the workers will be less likely
to enter into any discussions.

MINIBOSH.022411.Maximo Ginez III – mxginez3@yahoo.com

How long should they last?
A toolbox safety meeting normally lasts approximately 10
minutes if workers are not seated, if the topic is well chosen, and if
the discussion does not get off on a tangent. In preparing for a
meeting, and it appears that the interest will last more than 10
minutes, consider a series of 3 or 4 meetings or hold the meeting in
a room where the workers can sit comfortable and where noise and
other distractions are absent.

5. Ask questions about the topic to generate discussion. Allow time
for discussion and questions. If you don’t know the correct
answer, don’t guess at it. Ask for help from the Authorized
Safety Inspector / Safety Superintendent / Safety Coordinator.
6. Point out things that are being done right as well as problem
areas.
7. Use a real accident case to emphasize a point. The more recent

hat is a Job Hazard Analysis?
A job hazard analysis is a technique that focuses on job
tasks as a way to identify hazards before they occur. It
focuses on the relationship between the worker, the task,
the tools, and the work environment. Ideally, after you
identify uncontrolled hazards, you will take steps to

MINIBOSH.022411.Maximo Ginez III – mxginez3@yahoo.com

eliminate or reduce them to an acceptable risk level.

workplace injuries and illnesses by looking at your
workplace operations, establishing proper job procedures,

and ensuring that all employees are trained properly.
One of the best ways to determine and establish proper
work procedures is to conduct a job hazard analysis.

What is the value of a job hazard

Review of Accident History and Select Jobs to Analyze
Step
A job hazard analysis should be done first for jobs with the highest
1 injury rates. Look at your injury and illness reports to find likely
candidates. Also, jobs with “near misses” should be given
importance.
Involve the Employee and Do the Job Hazard Analysis
Step After you select a job for analysis, let the employee who is doing the
2 job know what you are doing. Point out that you are studying the job
itself, not checking up on the employee. Involve the employee

MINIBOSH.022411.Maximo Ginez III – mxginez3@yahoo.com

throughout the analysis. You will be looking at a job several times as
you do this analysis. Discuss and brainstorm for ideas to eliminate or
control those hazards.
Set priorities for hazardous jobs by Listing and Ranking
Step
List jobs with hazards that present unacceptable risks, based on
3 those most likely to occur and with the most severe consequences.
These jobs should be your first priority to solve.

Outline the solutions, document and implement

Step
List down all possible solutions to eliminate the hazard or risk, fill in
4 the JHA form, sign and submit for approval from the higher
management before implementation.

How do I identify workplace hazards?
A job hazard analysis is an exercise in detective work. Your goal is
to discover the following:
• What can go wrong? What kind of danger?
• What could be the consequences or outcome?
• How will it happen?
• What are possible contributing factors?

MINIBOSH.022411.Maximo Ginez III – mxginez3@yahoo.com

• How likely is it that the hazard may occur?

Classify the hazard according to its type:
