Why take this survey?

Your opinion will help teachers, principal and the administration to
determine what is being done well and areas in which there may be room
for improvement. We would like to know your opinions on how well our
school has met your family's and child(ren)'s needs and how you feel
about your involvement at the school. There are no right or wrong
answers. We are interested only in your opinions. Your answers will be
kept private.

How to take this survey?

If you have more than one student attending this school, think about your
students' common experience as you complete the survey. Please
respond to all items, if possible. You just have to mark either yes or no. If
