of Brgy. Bangad, Santa Maria, Isabela, after having been duly sworn to in
accordance with law, hereby depose and say that:

1. That I am the same person executing this affidavit;

2. That I am the mother of MA. THERESA B. LAGUYO who was born on

December 19, 1988 at Santa Maria, Isabela;

3. That there was an erroneous entry in her Certificate of Live Birth and I
am filing a petition for correction of entry in her Certificate of Live Birth;

4. That I am a unemployed, thus I belong to one of the indigent persons in

our barangay;

5. That I am executing this Affidavit to attest the veracity of the foregoing

statements and for whatever legal purpose it may serve.

This is to certify that MR. JESUSO BALINGAO MAGUNDAYAO, of legal age,

married, Filipino and a resident of Cansan, Cabagan Isabela a barangay kagawad of
Cansan Cabagan Isabela since December 2010 up to the present. Further he intend to go
Ireland as a tourist and he is expected to return to the country after three months.

This certification is issued on November 17, 2011 at Cabagan Isabela upon the
request of Mr. Jesuso Magundayao for whatever legal proposes it may serve him.

 I, ISLASH B. DATUL of legal age, married, Filipino and resident of Cubag, Cabagan
Isabela, after having been duly sworn to in accordance with law do hereby depose and say:

1. That I am the same person executing the foregoing affidavit;

2. That I was in Manama, Kingdom of Bahrain on April 2009 for the second time to
work as waiter;

3. Due to the chaos occurred in the said place, I am force to go back to the Philippines
on March 30, 2011 for a temporary vacation and for my safety;

4. And now, with my own consent I voluntarily decide to go back to Bahrain in order to
continue my contract at the Diplomat Radisson Blu Residence and Spa as a waiter;

5. I am executing this Affidavit in order to attest to the truthfulness of the facts

mentioned above and for the purpose of complying with the requirements of
Philippines Overseas Employment Agencies.

6. Further, affiants sayeth naught.
