Management Report
INTRODUCTION:-
This report provides comprehensive information regarding hazards which were identified
during inspection of WAQAS MAINTENANCE SERVICE WORKSHOP and its
surroundings on 27th September 2015  to meet the requirements of the NEBOSH IGC3
practical examination.WAQAS MAINTENANCE SERVICE WORKSHOP is
involved in engineering related works, including chemical activities, painting of
equipments, maintenance of heavy equipment (e.g. Heatexchangers ,compressors, tanks,
tube bundles, etc.)  machinery involves , truck, crane, hydraulic machinery, welding,
cutting,  electrical equipment like drill and arc welding plant, etc. The premises were
occupied by up to 200 staff members at one time involve in different activities.Staff work
is comprised of day and night shift. The major hazards which have been identified and
recommendations are made to avoid personal injuries to workers and property damages
are discussed below.

EXECUTIVE SUMMARY:
It is really appreciable that company showed interest and allowed inspection for the
improvement of health and safety system. After completion of the inspection it was found
that the organization has set up a proper HSE policy and HSE management system   in
the workplace. There were several areas of concern, but at the same time some good
observations were also made in the workshop and its surroundings. This shows that there
is only need of minor improvement and some training for all personnel about working
safely and keeping the surroundings safe since it can develop good safety culture among
all workers and staff.
Some of the observations like use of modified / handmade tools can cause injury , poor
ventilation in workshop  can cause suffocation, throat irritation, cough, sneezing and
trachea irritation. Damage lock of workshop’s door can cause cuts and bruises. Damaged
furniture in workshop can cause slips and trips and  serious injury like head injury etc.
Damage handle of baby grinder may cause hand injury like cuts and also vibration
hazards. My recommendation is to provide only minor financial support in order to cope
such type of unsafe acts in the working area.
In conclusion of this report I have mentioned that with a minor concentration of
management on health and safety rules and regulations which include Health and Safety
at Work Act 1974 (HSWA), Provision and Use of Work Equipment Regulation 1998, the
International Labor Organization (ILO), Occupational Safety and Health Convention (C
155) and by following some other relevant rules can drastically reduce the accident rate
within the organization. Good health and safety management system and its
implementation help to run the business smoothly, improve the moral of the workers as it
shows the management commitment, increase the productivity.Good health and safety
management system is less costly than the huge cost on rehabilitation after accident.
Good health and safety system to avoid legal penalties and meet the requirement of
international standards. A detailed survey report with findings and its control measures
are attached. Some points are required to be addressed to management, which needs
attention to avoid any accident.

Main Findings of Inspection:-


                Electrical Cables issue(04,12):
Electrical safety of work requires that all electrical equipments must be inspected and
always keep in good working order. It is duty of electricians working in workshop to
check electrical cables and tools for any damage. Damage and faulty equipment at
workplace   is the breach of ILO occupational safety and health convention C 155 Article
16(1) and Electricity at work regulation 1989 UK which say (No electrical Equipment
shell be put into use where its strength and capacity may be exceed in such a way as may
give rise to danger) UK REGULATION 05. All damage spectacles must be checked one
of the observations regarding damage cover of female Spectacle as it the requirement of
Management of health and safety at work regulation 1999 (13).So replacement of damage
tools & equipments should be priority as it is mention in provision and use of work
equipment regulation 1992. The financial cost of these equipments is too much lower
than the cost of accident an organization will bear after the accident occurrence due to
presence of damage electrical equipment. The severity of the electrocution is very high it
can cause skin burn by direct contact, burn the tissues infect the heart beat and breathing
system and if the duration of electrocution is long and the path of current into body
contain any systematic organ like heart lungs kidney then it would lead to fatality.
Health and Safety Management issue(06,07):-
A couple of lapses were visible leading to poor health and safety which could easily be
overcome by members of staff and at a very low cost to the organization. I have already
mentioned in recommendation about the effective cost measures. These poor safety
management acts or the breach of management of health and safety at work regulation
1999 it required employer to carry out risk assessments, make arrangements to implement
necessary measures and appoint competent people to arrange appropriate information
about housekeeping and cleaning and also breach of HSWA 1974. Breach or these laws
can cause of tripping hazards which have the consequence of cut bruises abrasion and fall
can cause head injury. Management should conduct training for all workers to cope with
these issues.
Housekeeping Issue(13 &26):-
Bad housekeeping condition was found in carpenter working areas  including scattering
of nail on the floor, no proper management for the placement of the nail which may
puncture in foot or other body part ,cause severe infection if contact.
Dustbin was overflowed that may not only lead to fire hazard in the presence of any
ignition source also give rise to unhygienic problem due to presence of pathogenic
bacteria cause biological infection. Existence of housekeeping plan and people required
hiring of adequate sweepers and providing them sufficient resources significantly
enhance housekeeping standard in workplace.
Chemical ,paints Storage  & Handling Issue (1,5,22):-
Open spray bottles under wash basin kept in area & Uncovered chemicals cans ,handling
with paints and volatile chemicals  without respirator, absence of MSDS are the breach of
rules and regulation included chemicals hazards information and packing for supply
regulation 2002  which required to suppliers classify , label and package dangerous
chemicals and provide safety data sheets for them and also dangerous substances and
explosive atmospheres regulation 2002 .If these chemicals are not properly cover they
will pose negative impact on worker health .If workers are continuously being exposed to
these chemicals can cause acute and chronic disease  including dermatitis, cancer, eye
irritation. Some chemical spontaneously react in the presence of sunlight which generate
fume or   if mix with each other by mistakes it can cause  chemical reaction which release
the toxic fumes. Company should introduce a safe handling  and safe procedure
according to material safety data sheet and provide MSDS of each chemical also provide
sufficient protective equipments including RPE while handling and training to worker.

Personal Protective Equipment Issue(01,02,  & 22):


The Personal Protective Equipment regulations state that all PPE must be suitably stored
and maintained. It is the legal responsibility of the employer to provide Personal
protective equipment at work regulations 1992  to their employees. During inspection I
found that worker dealing with paints without wearing gloves and respirator may lead to
dermatitis and pose other adverse effect on his health, grinder man was doing grinding
without using face shield .Another workers was kept exposing to  noise of generator may
cause of loss of hearing of temporary or permanent threshold or tinnitus, stress and may
reduce productivity by provision of PPE like earmuff or earplug to him  can protect him
from the exposure although it is a temporary recommendation but at least with this small
cost the company can protect his employees on the other hand in case of hearing loss of
employee the company may face to the enforcement action and compensation claims as
well as insurance expenses that is much more then the cost that occur on removal of this
unsafe act. On the other hand it will also enhance the moral of the employees, increase
their trust on the management .These factors enhance the reputation of the company and
good will.  

Work  Equipment Issue  (19 & 12):-


The Provision and Use of Work Equipment Regulations (1992) state that employers must
ensure that all work equipments are suitable, maintained and inspected. Use of modified
or handmade tools is not allowed to use

this type of unsafe act by workers. The cost of the tool is quite low as compare to the cost
of that the employer need to pay in case of accident from this hand made tool. The other
hidden cost will also relate in case of accident include the loss of trust on management
and absence of management commitment also reduce the moral of employees.  A similar
situation has arisen regarding broken handle and inappropriate rating of disc of baby
grinder.may lead to severe injury if broken and absence of handle reduces the grip of
operator may lead to disease associated with vibration hazard.  The employees have
shown their concerns about not being able to use the tool and it is in serious need of
replacement.  This is also creating an issue which must be addressed.
Manual Handling Issue(17):-
Any engineering workplace cannot run without lifting, handling, carrying, transferring of
object. Some issues are found regarding manual handling lifting of heavy cylinder,
improper handling of bulky metal sheets and other heavy material handled manually
without safe lifting technique and without wearing appropriate Personal Protective
Equipment. This is the clear breach of the Manual Handling Operations Regulations
1992.
Improper technique of manual handling may result MSDs, cuts, abrasion, fractures and
other indirect cost will be faced by company. According to my suggestion these heavy
materials must be transported or shifted correctly by using suitable mechanical equipment
to prevent injuries from bending and lifting under competent supervision. This will
prevent high costs through RIDDOR incidents and loss of productivity.
Conclusions:
After grasping a complete picture the enlighten cons and pros regarding inspection of
workplace .Health and safety inspection identified many unsafe acts and unsafe
conditions due to tremendous load of work it was looking that company is giving more
priority to production rather than health and safety  , What I believe a number of hazards
found due to lack of awareness, training and supervision which can be sorted out by
hiring competent people or providing  appropriate training  to workers  that can only be
possible by little management commitment .General health and safety matters due to
which there were several areas of concern which lead to breaches of

The areas which require management concentration are discussed in main finding section.
These are related to use of damage electrical equipment, failure of H & S management at
some places, risk from poor housekeeping, Risk from poor storage, chemical, paint and
there handling, PPE issue ,risk from damage work equipment and Manual handling issues
and necessary control to minimize risk disused in given table with time scale and require
budget is also mentioned. It is the moral responsibility of the employer to protect the life
of the worker because accident cause pain and suffering not only for the victim itself but
also for the friend and family members of the victim. An accident free environment is
highly desire in each and every industry, by meeting law employer can save himself from
any legal fine compensation claims and enforcement action. Good   H&S culture helps to
achieve the target and aim and objective of the organization. Maintaining good health and
safety plays important role to protect from financial loses .Good Standard of  Health and
safety helps to enhance  the productivity and increase the communication from top to
bottom and bottom to top also  increase the communication cooperation and competency
of the worker. If the identified observation will not be taken serious your organization
could face tremendous losses.
