Online Complaint Report for Violations of Executive Order
63 (Face Coverings) and Executive Order 67 (Safer at
Home: Phase Three Easing of Certain Temporary
Restrictions)
If you have observed violations of Executive Orders 63, 67, or 68 and wish to file a complaint, fill out this report form
as completely as possible. Executive Order 63 involves the requirement to wear a face covering while inside
buildings. Frequently Asked Questions about Executive Order 63 are available here. Executive Order 67 Safer at
Home: Phase 3 eases certain temporary restrictions due to COVID-19; complaints related only to Executive Order 67
that involve cloth face coverings or capacity requirements may be filed with this form. Executive Order 68 involves
addiditional restrictions in the eastern region.
The Virginia Department of Health (VDH) has the authority to enforce Executive Order 63 and section A of Executive
Order 67. VDH will review your complaint and forward it to your Local Health Department.
Complete one report for each location where the incident occurred. Please be advised that any information you
provide may be subject to the Freedom of Information Act, Virginia Code Section 2.2-3700, et. seq.

morning of 11/17/20: "I am an employee at Little
Lambs Christian Dayschool. I have been informed
that they are not telling the truth about a
positive COVID case. I wanted to give some details
about that if you could give me a call back."
