Nel gruppo delle malattie reumatiche infiammatorie, la SpA è la diagnosi più comune dopo
l’Artrite Reumatoide. La sua prevalenza è stata a lungo sottostimata. La SA e le Spondiloartriti
Indifferenziate (SA) sono i sottogruppi più comuni nei paesi occidentali, con una prevalenza
compresa tra 0.2 e 1.2 % (2).
La SA ha un'incidenza 3 volte maggiore nel sesso maschile ed esordisce in genere in
soggetti giovani con età compresa tra i 20 e i 40 anni.
La frequenza della SA varia enormemente tra le diverse razze. Per esempio, SA colpisce circa 2
su 1000 Caucasici,
La prevalenza della spondilite anchilosante varia dallo 0,01 allo 0,5%, con un valore per
l'Italia dello 0.05%. Colpisce in genere soggetti in età giovanile (15-35 anni) con netta
prevalenza, almeno nelle forme più classiche, dei maschi rispetto alle femmine
Artrite Psoriasica
La psoriasi cutanea e una malattia piuttosto frequente con una prevalenza nella popolazione
bianca del 2-3%. In ITALIA la psoriasi coinvolge il 3-4% della popolazione (circa 1Ml di
Italiani). Piu del 10% dei pazienti con psoriasi presenta artrite, sacroileite o spondilite. La
psoriasi esordisce generalmente in un'eta compresa tra i 30 ed i 50 anni e nel 15% dei casi
precede l'insorgenza delle manifestazioni articolari. Generalmente si distinguono due tipi di
psoriasi, il tipo 1 che esordisce precocemente (<40 anni) con una forte associazione familiare e
presenta una forte associazione con antigeni del complesso maggiore di istocompatibilita ed il
tipo 2 ad esordio tardivo che non presenta associazione familiare
Spondyloarthritis is the name of a family of rheumatic diseases that are characterized by arthritis of
the spine and peripheral joints, inflammation in the area where ligaments and tendons attach to
bones, and inflammation of the skin, intestines and eyes. They are classified as seronegative
spondyloarthritis to differentiate them from rheumatoid arthritis: symptoms between the two
conditions often overlap, but blood tests in patients with the former are negative for the RhF marker
(rheumatoid factor). The exact cause of spondyloarthritis is unknown, but hereditary factors are
thought to play an important role, since these illnesses often run in families, and especially in
individuals who express a genetic markers called HLA-B27.
Do you know why Italian dermatologists prescribe more cyclosporine than methotrexate. Are there
any guidelines which recommend the usage of cyclosporine prior to methotrexate? Or do you know
other reasons?”
Medico#1:
è molto indicato nelle linee guida nazionali (forse 2010). Gli effetti collaterali sono molto gestibili.
È usato come prima scelta e se c’è un aggravamento della malattia si “switcha” al MTX

farmaco indicato nelle linee guida nazionali (le L.G. indicano 4 farmaci: mtx, ciclosporina,
acitretina e PUVA)
la risposta clinica nelle lesioni è rapida, cioè entro le 3/4 settimane
in molti pazienti con recidiva, uno studio nazionale ha dimostrato che per cicli brevi (massimo 12
settimane) seguito da un’interruzione brusca e una successiva ripresa della terapia, consente un
controllo totale della malattia senza eventi avversi. Si è in grado di ottenere una clearance delle
lesioni con effetto prolungato nel tempo
È meno tossico del MTX, la ciclosporina incide solo sul fegato

We have spoken with some Italian dermatologists and this is what came out about the prescription
of cyclosporine vs methotrexate:
At the moment Cyclosporine is the drug indicated in the National Guidelines for the treatment of
moderate and severe PsO.
- it has light side effects and is perceived as less toxic than methotrexate
- the clinical response of the lesions is fast, within 3 /4 weeks from the treatment start
- a National study has demonstrated that in relapsing patients after a short cycle (max 12 weeks) of
treatment followed by an interruption and a subsequent second cycle of treatment it allows an
excellent control of the disease without adverse events with long-term effects
For these reasons Cyclosporine is often used as first line/ first choice treatment vs methotrexate,
which is usually used as following line if the disease gets more severe.

2)Reactive arthritis: it presents with redness and swelling of the large joints following a bacterial
inflammation of the gastrointestinal tract, often caused by food poisoning, or genitourinary tract,
usually caused by the micro-organisms responsible for chlamydia and gonorrhea. The genetic
marker is found in 60-80% of patients.
3)Psoriatic arthritis and psoriatic spondylitis: it occurs in about 5% of people with psoriasis and
is linked to the HLA-B27 marker, which is found in 60% of cases. The disease often manifests itself
with tendinitis and swelling of the fingers, and tends to develop 10 years after the onset of psoriasis.
4)Enteropathic arthritis: it is associated with inflammatory bowel diseases, ulcerative colitis and
Crohn's disease. In patients with enteropathic arthritis, inflammation of the peripheral joints is
accompanied by gastrointestinal symptoms such as abdominal pain and diarrhea. About 60% of
patients have the HLA-B27 marker.
