Note: The trainee and the supervisor must have a copy of this form. The column for rating maybe used either by giving
a numerical rating or simply indicating competent or not yet competent. For purposes of analysis, you may require
industry supervisors to give a numerical rating for the performance of your trainees. Please tak

Title : Prepare Trainee’s Record Book
Performance Objective:
In reference to the output of the previous activity which is the
Training Plan, prepare your Trainees Record Book following suggested
Template.
Steps/Procedures :
1. Secure a copy of the CBC of your qualification;
2. Prepare the template for each competency of your qualification.
It is recommended that you make a TRB for the qualification as
a template.
3. List the learning outcomes for each competency;
4. Secure a copy of the Training Plan you accomplished in
previous Job Sheet.
5. To make a TRB for a particular trainee, copy the training
activities identified in the training plan.
6. Using hypothetical data, try to accomplish TRB for at least one
competency.
7. Evaluate your output using the Performance Criteria Checklist.
