P&I INCIDENT – DANGEROUS GOODS CONTAINER OVERBOARD

A. INTRODUCTION

This study is all about the analysis of the incident of P&I – a dangerous goods container

overboard that occurred while discharging containers from a vessel at anchorage when a

container slide over the pontoon of the barge and overboard.

B. MAIN PROBLEM

The main problem in this study base on the report is that the operator of the crane did not

take a good care of handling the container operating the ship’s crane and he hit the other

container then it slide over the pontoon of the barge and overboard and sank through the bay.

C. OBJECTIVES

 To gain knowledge about the proper handling in operating a ship’s crane.

 To know the responsibilities when it comes in operating a ship’s crane.

 To deliver container from the ship to the barge without any casualties.

 To be aware of the casualties when sinking a container whether it is dangerous cargo or

not.

D. AREAS OF CONSIDERATION

- Internal / External Factors

a) Internal Factors

 Strength – Operators of the ship’s crane will be more aware of properly handling the

ship’s crane after the incident that happened.

 Weakness – Operator that is the reason of the incident will be pressured and will face

the casualties. That incident can affect the operator’s work when he keeps thinking of

it.

b) External Factors

 Strength – The Company of the ship will help the person that is cause of the incident.

 Weakness – Crew’s contract may be hold because of the incident. Either the company

or the person cause of the incident will pay the casualties.

E. COURSES OF ACTION / STRATEGIES

 Keep the crane maintained or to check for defects after long periods of use.

 Ensure to operate the crane with patience and extreme caution.

 Assign proper assistance or spotter for the operator of the ship’s crane.

 Make sure the crane is not overloaded and that the cargo is properly balanced.

 The operator of the crane should be well-trained or well-experienced.

F. FINAL DECISION / RECOMMENDATION

It is recommended to keep the crane maintained or to check for defects after long periods

of use to avoid malfunctioning that can cause accidents. Operate the crane at a safe speed with

patience and extreme caution to ensure the safety operation of the crane. Proper assistance or a

spotter is one of the important factors when operating a ship’s crane as the operator can’t see

clearly what is happening outside. Operator should make sure that the crane is not overloaded

and that cargo is properly balanced so that he can control the crane easily to avoid accident.

Lastly, it is highly recommended that the operator of the crane should be well-trained or well-

experienced as he knows how to operate the crane well and can finish the operation safely.
