III. PROBLEM PRESENTED

The client is requesting for financial assistance that was diagnosed with Community
Acquired Pneumoni Moderate Risk; Soft Tissue on Pelvic Area; R/O Pelvic Mass R/O Bone
Metastasis at H.M. Corachea General Hospital. She claimed that they are having difficulty
sustaining the medical assistance in connection they are asking for outside help.

VI. EVALUATION/RECOMMENDATION
Client is found for financial assistance due to the following findings:
1.) The meager income of the family is insufficient to sustain the family needs other
household expenses.
2.) They have no other source of income or asset to tap in time of crisis.
3.) They cannot seek assistance from the nearest relatives for they are also poor.

In view of the above, may we recommend the client for financial assistance to help the
client to support his/her medical needs.
