AFFIDAVIT OF LOSS

I, CHRISTIAN JAY PEDRO, of legal age, Filipino and a resident of

Brgy. 52 B Lataag, Laoag City, Ilocos Norte, Philippines, after having
been duly sworn to in accordance with law hereby voluntarily depose and
state: That-

1. I am an owner/holder of an unregistered power chainsaw

with Serial No. ___________________;

2. I bought the said power chainsaw sometime in

_______________;

3. Upon purchase, I was issued an Official Receipt as evidence

for my purchase;

4. I always keep the said Official Receipt on top of my personal

cabinet inside our house;

5. On May 27, 2018, I discovered that the said Official Receipt

was no longer in the place where I kept it;

6. I exerted diligent effort to locate the Official Receipt but to no

avail. Hence, the said Official Receipt is now deemed lost; and

7. I am executing this affidavit to attest to the truth of the

foregoing facts and for whatever legal purpose it may serve.
