This form is for your reference only. Actual answers to the entry form questions and images requested
must be entered directly online. We highly recommend typing your answers in this document first, then
copying and pasting into the online submission site to prevent accidental loss of work. There are
character limits for each question given in parentheses. Use “Character Count WITH spaces” to verify,
because you will not be able to exceed these limits. Do NOT include your design firm name anywhere on
this form, or your entry will be disqualified. Consider using bullet points for clarity.

accompanies your design in all print and online coverage. Please avoid using industry jargon or
