WHEREAS a complaint (for information) has been filed on the 26th day of December,
1990, in the above-entitled COURT, charging the above-mentioned defendant with the offense of
THEFT was admitted to bail in the sum of Six Hundred Pesos (P600.00).

NOW, THEREFORE, I, KUHA TINDA @ DUKOT of Brgy. Mapayapa, Dalupiri Island,

Cagayan Province accused, hereby undertake that I will appear and answer the charge above-
mentioned in whatever court it may be tried, and will at all times hold myself amenable to the
ordered and processes of the Court and if convicted, will appear for judgement and execution
thereof, if I fail to perform any of these condition I will pay Republic of the Philippines the sum of
P600.00 under O.R. NO. 0351618.

It is further stipulated that pursuant to Sec. 19, Art. IV of the Constitution and letter of
Instruction No. 40 of the President of the Philippines, dated No.10, 1972, I the undersigned
accused, hereby agree that in case I jump bail of fails to appear for trial without any justification
despite due notice to me, my absence will be deemed and express waiver of my right to be
present, which will warrant the Court to proceed with the case as if I was present.
