AFFIDAVIT OF UNDERTAKING

I ________________________________ hereby abides the COVID 19 Health and Safety protocols in

compliance with DTI and DOLE as such I am expected to wear the proper PPE ( Personal Protective
Equipment ) to avoid the spread of virus I understand that I should demonstrate proper store
routines in wearing gloves, face mask , hairnet ,and face shield at all times.

I commit myself to perform all duties and responsibilities assign to me.

I will be responsible to report to management any fellow employees not observing safety protocol and
having health symptoms, health issues during at work.

I agree to provide the necessary discretion by the management to rotate work schedules put on hold or
suspend work schedules. This is particularly to make sure that any crew having one or several symptoms
will be required to be quarantined at home and reported to DOH. I agree if required by management to be
subjected to any covid rapid test internally or by LGU and refusal or failure to participate will result to
suspension of work schedule.

I release K Trio Food Food and Management Services Inc. from any liabilities arising from the Covid 19
health and financial impact to me in my work conditions and limitations under the new normal and IATF
categories imposed on provinces and or specific locations.

Any dmage in kitchen equipments, covid personal protective equipments lost of utensils gadgets,scanner
due to negligengence,cash over/shortage of more than 20 pesos ( automatic na ikaltas sa sahod po sir)

Relocated where there is immediate requirements during Havi deliveries, Awol employees where in
immediate and short and long term designation to a specific work station and store that requires my
presence to substitute or replace temporarily the requieremnts and for reason of security during the
investigation of pilferage and other illegal activites including dispute among employees.

I understand the legal implication that should I ever violate this valid legal directive, I will be subject to
disciplinary measures which may result in the outright termination of my employment on the grounds of
insubordination, willful disobedience of the lawful order of my employer, willful breach of trust reposed
upon my position, and/or negligence.
