Therapeutic uses of Opioid analgesics/ Morphine:
1) Pain: acute MI, fracture, dislocation, burns, postoperative pain
and cancer pain.
Q. Why morphine is given in acute pulmonary edema?
2) Acute pulmonary edema: morphine provides dramatic relief
from dyspnoea due to pulmonary oedema associated with LVF.
the probable mechanisms are as follows:
a) Venodilation and reduced preload, followed by fall in
afterload.
b) reduced sensitivity of respiratory centre to elevated CO2
levels
c) antianxiety effect and reduced perception of shortness of
breath.

4) diarrhea
5) anaesthesia & preanesthetic medication
Adverse effects:
2) pinpoint pupil
4) stupor/ coma
a) Extremes of age

OPIOID AGONIST/ANTAGONISTS AND
PARTIAL AGONISTS:
Pentazocine:
Pharmacological Actions:
The pattern of CNS effects produced by pentazocine generally
is similar to that of the morphine-like opioids, including
analgesia, sedation, and respiratory depression. The analgesic
effects of pentazocine are due to agonistic actions at κ opioid
receptors.
Higher doses of pentazocine (60 to 90 mg) elicit dysphoric and
psychotomimetic effects.
The cardiovascular responses to pentazocine differ from those
seen with typical μ receptor agonists, in that high doses
cause an increase in blood pressure and heart rate.
Pentazocine acts as a weak antagonist or partial agonist at μ
opioid receptors.
Tramadol:
Synthetic codeine analogue, weak μ agonist and analgesic agent.it
cats on central opiate receptors (inhibition of uptake of NAdr and
enhanced serotonin release) to produce analgesia. Undergoes first
pass metabolism. No respiratory depression on therapeutic dose and
less typical opioid side effects. Does not produce drug dependence
or tolerance.

Naloxone :
It is a competitive antagonist (pure/ true) at opioid receptors,
notably the μ and κ receptors but no agnostic activity.
