NONDESTRUCTIVE TEST PROCEDURE

PERSONNEL QUALIFICATION AND CERTIFICATION

1.0 SCOPE

1.1 This specification applies to personnel performing nondestructive testing on P&H Mining Equipment
product.

2.0 PURPOSE

2.1 This specification outlines the requirements necessary for qualification and certification of
Nondestructive Testing personnel by P&H Mining Equipment.

2.2 This specification is based on the Recommended Practice No. SNT-TC-1A (published by ASNT) and
establishes the written practice of P&H Mining Equipment.

3.0 REFERENCES

3.1 ASNT Recommended Practice No. SNT-TC-1A (1980 and later editions)

3.2 ASNT Level III Certificate By Examination, ASNT-CP-3-92

4.0 LEVELS OF QUALIFICATION AND CERTIFICATION

4.1 There are three levels of certification, Levels I, II and III. These levels are divided according to the
acquired skills of the individual.

4.2 Prior to certification and during qualification of an NDT Level I individual, the candidate must be
considered a trainee. A trainee shall work with a certified individual and shall not independently
conduct any NDT, interpret or evaluate the results of any NDT, or report NDT results.

4.3 NDT Level I - An NDT Level 1 individual shall be qualified to properly perform specific calibrations,
specific NDT, and specific evaluations for acceptance or rejection determinations according to written
instructions and to records results. The NDT Level I shall receive the necessary instruction or
supervision from a certified NDT Level III individual or designee. Designee is an individual selected
and deemed qualified by a certified NDT Level III.

4.4 NDT Level II - An NDT Level II individual shall be qualified to set up and calibrate equipment and to
interpret and evaluate results with respect to applicable codes, standards, and specifications. The
NDT Level II shall be thoroughly familiar with the scope and limitations of the methods for which
qualified and shall exercise assigned responsibility for on-the-job training and guidance of trainees
and NDT Level I personnel. The NDT Level II shall be able to organize and report the results of
NDT.

4.5 NDT Level III - An NDT Level III individual shall be capable of establishing techniques and
procedures; interpreting codes, standards, specifications, and procedures; and designating the
particular NDT methods, techniques, and procedures to be used. The NDT Level III is responsible
for the NDT operations for which qualified and assigned and shall be capable of interpreting and
evaluating results in terms of existing codes, standards, and specifications. The NDT Level III shall
have sufficient practical background in applicable materials, fabrication, and product technology to
establish techniques and to assist in establishing acceptance criteria when none are otherwise

available. The NDT Level III shall have general familiarity with other appropriate NDT methods, as
demonstrated by the ASNT Level III Basic examination or other means. The NDT Level III, in the
methods in which certified, shall be capable of training and examining NDT Level I and II personnel
for certification in those methods.

4.6 Personnel outside P&H Mining Equipment, based on qualification, and experience may perform such
duties as delegated to a Level I, Level II or Level III individual in any one or more of the applicable
NDT disciplines. Such personnel must be qualified in accordance with SNT-TC-1A (1980 or later
edition).

5.0 EDUCATION, TRAINING AND EXPERIENCE

Candidates to be considered for certification in nondestructive testing shall be qualified with regard to
education, training and experience to ensure understanding of the principles and procedures in those
areas of testing in which they are being considered for certification. To be considered for certification to
Level I or Level II by P&H Mining Equipment, a candidate shall satisfy the following criteria for the
applicable NDT method and level.

Note 1: Grammar school graduation or demonstrated proficiency.


Note 2: High School graduation, diploma or equivalent.
Note 3: Completion with a passing grade of at least (2) years of engineering or science study at a university,
college or technical school.
Note 4: Level II requirements are in addition to those of Level I. If qualifying directly to Level II, candidate
must meet the sum of both Level I and Level II requirements.

5.1 Level III - All Methods

To be considered as a candidate for NDT Level III by P&H Mining Equipment, the candidate must fulfill
one of the following criteria:

5.1.1 Have graduated from a minimum four year college or university curriculum with a degree in
engineering or science plus one year of experience in nondestructive testing in an assignment
comparable to that of an NDT Level II in the applicable test methods, or

5.1.2 Have completed with passing grades at least two years of engineering or science study at a
university, college or technical school, plus two years of experience in assignments at least
comparable to that of an NDT Level II in the applicable test methods, or

5.1.3 Have four years experience in an assignment at least comparable to that of an NDT Level II in
the applicable test methods, or

5.1.4 Have been certified as Level III by the American Society for Nondestructive Testing (ASNT)
and hold a valid certificate in the applicable test methods (Ref: ASNT-CP-3-92).

6.0 TRAINING PROGRAMS

Candidates being considered for certification shall complete the P&H Mining Equipment training program
to be established by the NDT Level III.

6.1 Prior documented training and/or experience outside P&H Mining Equipment will be considered by
the NDT Level III when determining certification eligibility of personnel.

6.2 Training programs will be conducted by the NDT Level III or designee.

7.0 EXAMINATIONS

NDT personnel, prior to certification must meet the applicable examination requirements of this written
practice. Examination requirements to verify the physical and technical qualifications of NDT candidates
shall consist of the following:

7.1 Physical

7.1.1 The candidate shall be given an examination to assure natural or corrected near distance
acuity in at least one eye so that he or she is capable of reading a minimum of Jaeger #1
letters at a distance of not less than 12 inches (30.5cm) on a standard Jaeger test chart.

7.1.2 The eye examination shall be administered by qualified personnel, such as a nurse or optician,
prior to certification, and on an annual basis thereafter. The results of the examination will be
kept on file for the period of certification.

7.1.3 Individuals shall also be capable of distinguishing and differentiating contrast between colors
used in the method for which qualified, as demonstrated by the practical examination or test
performance upon initial certification and at each recertification.

7.2 General (Written) - Levels I and II

7.2.1 The general examinations shall cover the basic principles relative to the test method for which
the candidate is to be certified.

7.2.2 The general examination questions may or may not, in total or in part, include those published
by ASNT.

The minimum number of test questions shall be as follows:

7.3 Specific (Written) - Levels I and II

7.3.1 The specific examination shall cover equipment, operational procedures and test techniques
that the candidate may encounter in his or her assignment.

7.3.2 The specific examination shall also cover applicable specifications, codes and acceptance
criteria relevant to the NDT method and the P&H Mining Equipment.

The minimum number of test questions shall be as follows:

7.4 Practical (Performance) - Levels I and II

7.4.1 The candidate being considered for certification shall demonstrate familiarity with and ability to
operate the necessary NDT equipment, record and analyze the resultant information to the
satisfaction of the examiner.

7.4.2 One or more selected specimens shall be tested. These specimens may be either pre-
selected or in-house normal production items. Performance will be graded by either the Level
III individual, or the designated representative. Grading will be based on at least (10) different
checkpoints requiring an understanding of test variables and procedural requirements.

7.4.3 The description of the specimen, the test specification, and the results of the examination shall
be documented and maintained as part of the candidates certification records.

7.4.4 Written examinations shall be administered without access to reference material except as
deemed necessary by the Level III. All questions used shall be approved by the Level III.

7.5 NDT Level III Examinations

7.5.1 Basic Examination (required only once when more than one method examination is taken).
The number of questions which shall be given is as follows:

a) Fifteen (15) questions relating to understanding the SNT-TC-1A document.


b) Twenty (20) questions relating to applicable materials, fabrication, and product technology.
c) Twenty (20) questions that are similar to published Level II questions for other appropriate
NDT methods.

7.5.2 Method Examination (for each method):

a) Thirty (30) questions relating to fundamentals and principles that are similar to published
ASNT Level III questions for each method, and
b) Fifteen (15) questions relating to application and establishment of techniques and
procedures that are similar to the published ASNT Level III questions for each method, and
c) Twenty (20) questions relating to capability for interpreting codes, standards, and
specifications relating to the method.

7.5.3 Specific Examination (for each method):

a) Twenty (20) questions relating to specifications, equipment, techniques, and procedures


applicable to P&H Mining Equipment product(s) and methods employed, and to the
administration of this written practice.

7.5.4 A valid endorsement on an ASNT NDT Level III certificate fulfills the examination criteria
described in para's 7.5.1 and 7.5.2 for applicable NDT method.

7.5.5 P&H Mining Equipment waives the specific examination described in para. 7.5.3 a) if the
candidate has a valid ASNT NDT Level III certificate in the method and if documented
evidence of experience exists, including the preparation of NDT procedures to codes,
standards, or specifications and the evaluation of test results.

8.0 GRADING OF EXAMINATIONS

8.1 An NDT Level III shall be responsible for the administration and grading of examinations for NDT
Level I and II personnel. The administration and grading of examinations may be delegated to a
designated representative of the NDT Level III and so recorded. The actual administration and
grading of Level III examinations may be performed by a designated representative of the P&H
Mining Equipment.

8.2 For Level I and II personnel, a composite grade shall be determined by simple averaging of the
general, specific, and practical examination results. For Level III personnel, the composite grade
shall be determined by simple averaging of the basic, method, and specific examination results.

8.3 Examinations administered for qualification shall result in a composite grade of at least 80 percent,
with no individual examination having a grade less than 70 percent.

8.4 Those failing to attain the required grades must wait at least thirty (30) days or receive suitable
additional training as determined by the NDT Level III before re-testing. Candidates for certification

will be allowed (1) retest if the initial test was unsuccessful. If the retest is unsuccessful, candidates
must wait for a period of (1) year and show evidence of additional training before attempting
certification.

9.0 CERTIFICATION

9.1 The qualification and certification of all levels of NDT personnel is the responsibility of the P&H
Mining Equipment.

9.2 Certification records shall be maintained on file by Quality Assurance and shall contain all information
pertinent to the individual's qualification and certification.

9.3 During the process of qualification, experience may be gained simultaneously in two or more
methods if:

a) the candidate spends at least 25% of work time on each method, and
b) the remainder of work time is spent in NDT related activities (includes VT)

9.4 P&H Mining Equipment certification shall be deemed revoked when employment with the company is
terminated.

10.0 RECERTIFICATION

Recertification by examination is required if an individual has not been active in a particular NDT method
for a period of time exceeding (12) months. NDT personnel may be reexamined at any time at the
discretion of the P&H Mining Equipment and have their certification extended or revoked. All levels of
NDT personnel shall be recertified periodically in accordance with the following:

10.1 Level I and II

10.1.1 Re-examination every (3) years with a maximum (3) month grace period. A "specific"
examination (para. 7.3) will be administered for each method. A minimum grade of 80% is
required for recertification.

10.1.2 Candidates who fail to achieve the 80% minimum grade level will be given at least (2) hours
of additional training for each method examination and allowed to retest within (30) days.

10.1.3 Failure to achieve the minimum grade following the above retest will result in loss of P&H
Mining Equipment certification for each method involved. A waiting period of at least (1)
year will be required, along with evidence of additional training, before attempting
recertification.

10.2 Level III

10.2.1 Re-examination every (5) years with a maximum (6) month grace period. Re-examination
shall be in accordance with Sections 7.0 and 8.0.

10.2.2 Candidates who fail to achieve the minimum grade in any method shall pursue additional
training and retest within (3) months.

10.2.3 Failure to achieve the minimum grade following the above retest will result in loss of P&H
Mining Equipment certification for each method involved. A waiting period of at least (1)
year will be required, along with evidence of additional training, before attempting
recertification.

10.2.4 Re-examination requirements are waived by P&H Mining Equipment if the candidate
maintains a valid Level III certificate issued by ASNT for each method required.

10.3 Certified NDT candidates who fail to achieve the minimum grade level for the initial recertification
examination, shall be allowed to continue normal duties in accordance with prior certification level
until retest requirements have been satisfied. Following retest, certification shall either be extended
or revoked.
