CONTRACT OF SERVICE

KNOWN ALL MEN BY THESE PRESENTS:

THIS AGREEMENT made and entered into this 13 th day of October, 2016 by

and between the MUNICIPALITY OF PALO, LEYTE represented by HON.

REMEDIOS L. PETILLA, Municipal Mayor hereinafter referred to as the First

Party;

and

LEYTE GULF TRAVEL AND TOURS represented by MARIA LOURDES

BERNADETTE V. RUIZ, owner, with office address at 39 Independencia Street,

c/o Calle Zaragoza, Tacloban City, Leyte, Philippines hereinafter referred to as the

Second Party;

WHEREAS, the First Party has engaged the services of the Second Party

to act as facilitator for the Local Tour Guiding Workshop for Local Tour Guides of

Palo;

NOW, THEREFORE, the First Party hereby enters into a CONTRACT OF

SERVICE with the Second Party, whereby the First Party engages the services

of the Second Party under the following terms and conditions:

1. The FIRST PARTY shall pay the SECOND PARTY the amount of FIFTY

THOUSAND PESOS (P50,000.00) as Professional Fee; and

2. In consideration of the above amount, the SECOND PARTY shall facilitate

during the Local Tour Guiding Workshop on October 14 – 16, 2016.

 The FIRST PARTY AND SECOND PARTY agree and warrant that they have

full understanding of the terms and conditions set forth in this contract. The

parties hereto agree that the proper Philippine Court shall have jurisdiction over

the case, and further, that the attorney’s fee equivalent to 25% of the disputed

amount shall be added to the cost of the suit as maybe deemed proper by the

court.

In witness whereof, the parties have hereunto signed this 13th day of

October, 2016 at Palo, Leyte, Philippines.
