NOTICE FOR EMPANELMENT.

Applications are invited from reputed Printers/ Firms/Vendors with at least 3 years standing & having PAN &
GST registration for empanelment with Life Insurance Corporation of India, Goa Divisional Office, Patto,
Panaji, Goa for the category of services as mentioned below.

The interested Printers/Firms/Vendors/Suppliers should apply in prescribed form (Annexure - A2) & is required to
be filled up for each category which may be downloaded from our website www.licindia.in (Tender Link). The
sealed envelope containing the application form & other relevant documents should be super-scribed as
“Application for empanelment of printer/supplier/service provider, SL. NO. ____, Name of the Category
________________________________________.” to be submitted to The Manager (O.S.) LIC of India, Goa
Divisional Office, Jeevan Vishwas, EDC Complex, Patto, Panaji - Goa — 403 001 ,along with non refundable
application fee of Rs. 295/ -( inclusive of 18% GST), for each category, in the form of Demand Draft/ Banker’s
cheque in f/ o LIC of India, payable at Panaji, Or by paying cash during cash hours (10.15a.m. to 3.15 p.m.) on
working days, except 2nd & 4th Saturday.
The selection shall be at the sole discretion of the appropriate authority of LIC of India, Goa Division. The
Corporation reserves the right to accept/ reject any/ all applications without assigning any reasons whatsoever.
Existing Empanelled Vendors / Service Providers are also required to apply again, for new Empanelment
Firms Blacklisted by any LIC office and PAN India should not apply.
