I certify by my signature that the information I have provided on this form is accurate and correct. I also understand that all my
entry materials, including photographs, as well as any additional photographs taken by Boone and Crockett (our representatives
or agents) during Awards Programs or Judges Panels, or likenesses rendered from these photographs become the property
of the Boone and Crockett Club and may be used to promote the Club, and its records-keeping activities.
In an effort to expand the value of the records data, we desire to obtain age data for all trophies. We also
wish to obtain rack weights of unmounted caribou, moose, and elk, and tusk weights for walrus. Adding age
data to the other measurements for trophies should help substantiate the contention that most trophy-sized
animals are well past prime breeding age. Weights of racks for caribou, moose and elk will aid in correlation
of our records keeping with those systems used in Europe for their representatives of these animals. Please
note that this is only a request for information; lack of this information for your trophy will not affect its
entry status.
A. AGE DATA: If your trophy was aged by a wildlife biologist of the state or province you took it in, please at-
tach a copy of the age determination. If age has not been determined, we would appreciate you for-
warding the appropriate material indicated below, if still available. Should the
requested material no longer be available, please indicate this on the line provided below.

B. RACK/TUSK WEIGHTS: The rack and attached skull weight for all unmounted caribou, moose, and elk
should be taken in pounds and ounces and recorded below. Please use the codes below to indicate
the amount of attached skull (circle the one that most closely describes the rack). For walrus tusks,
the weight of each loose tusk should be taken, to the nearest ounce. If your rack or tusks are mounted,
please check the box to the right. MOUNTED RACK/TUSKS
