Composition of the Leadership Team:

Process:

1. Call for Nominations


 City Health Officer calls for the opening of nominations;
 Entries/Nominations can be thru email (paranaque_cityhealthoffice@yahoo.com) or
brief memo (sent to admin office)
 Deadline of nominations until December 16, 2019 (5:00 pm)

2. Submission of supporting documents


 Detailed narratives from Immediate Supervisor/Unit Heads (see Form) and proof of
accomplishments
 Deadline of submission – December 17, 2019 (12:00 mn)
 Submit thru email (paranaque_cityhealthoffice@yahoo.com) or to admin office

3. Judging based on criteria:


 Judging will be conducted by Board of Judges thru document review of submitted
narratives;
 Board shall be composed of:
o Dr. Dionisio Sabio – Senior Assistant City Health Officer
o Dr. Ruben Ver Bombeta – Junior Assistant City Health Officer
o Dr. Marissa Banta – Head, Dental Division
o Mary Ann Malbas, RN – Head, Administrative Section

Modelled principled behaviour and sound business ethics. Earned the trust of others
through consistency of actions with stated values and commitments. Took
responsibility for one’s own actions. Showed having a strong personal sense of what

securing support and forming alliances with both internal and external stakeholders

uplifting of health of the members of the community.

Please fill out this form in order to support the nomination of your staff for the award. Please be as
extensive and detailed as possible. The narrative can be in paragraph or bullet form. Only English or
Tagalog will be accepted. You may adopt the methods of the CSC-SPMS guidebook. Accordingly, one
of the best ways by which performance is documented is through recording of Critical Incidents.
These capture specific events where employees performed exceptionally well. Written in STAR
format, it should provide details about the Situation or Task when the incident happened (with
dates/month within the year), the Action that was taken and the Result.

For the GEM of CHO Award (Going the Extra Mile), the narrative should give examples or evidence of
how your staff was able excel or perform more than what was expected from them while living-up to
our City Health Office Core Values:
