Fissure sealants

Definition of fissure sealant:

measures to prevent caries by clogging deep


pits and fissures with fillers / coatings.
Fissure Sealant

• Pit and fissure is a place


where plaque is hidden
and difficult to clean.

• This area is generally a


vulnerable area
against caries and is least
affected by fluorine.
Fissure Sealant

• Fissure cover is a material that


is designed to prevent caries in
the fissure and pit.

• This material especially used in the


occlusal area of ​the tooth to patch
occlusal fissures so that the hidden
areas that allow caries to appear
can be removed.
Clinical Indications.
1 Permanent Teeth
• Fissure closure should be done as soon as possible after
the tooth erupts. The first and second molars are usually the
main candidates .

• If the risk of caries is considered high then a


caries closure will be needed as part of a
comprehensive preventive program.
Clinical Indications.

2. Milk teeth
• Fissure closure on milk molar is not as frequent as in fixed
molar.

• The indications are especially if caries is found


everywhere in patients with a high caries risk

1. Isolation
• In relation to the
the success or failure of the fissure closing
attempt, isolation may constitute the most
critical stage .
• If the pores made by etching are
covered in saliva, the bonds formed
will be weak.
• Rubber isolator is a method of
isolation that is reliable and preferred
rather than use of cotton rolls and
saliva .

this is difficult to do well, because the


etched teeth must be washed cleanly.
Wet cotton insulator can not be avoided
so it must be replaced.

• At the time of this replacement, it is very easy


for the tooth surface to be etched to be soaked
in saliva and this contamination will damage the
bond between the fissile and the enamel.
2. Clean the teeth

• Oily pumis mix should not be used


because it will disturb the etching.

• The occlusal surface of the teeth is


polished with pumis . Then the pumis is
washed clean with a spray of air and
water, then a sharp sonde is dragged
along the fissure. This way to remove
plaque on deeper areas that cannot be
cleaned by brushing . Then the teeth are
washed again and dried well.
3. Eating

• Phosphoric acid used


is between 30-50%
for minimal tissue
loss, however

the maximum pores.

• The etching material is placed over the entire occlusal surface, and
the lingual or buccal whose groin needs to be covered. The etching
of the entire occlusal surface avoids the danger of fissile material
covering the non-etched area, causing leakage.

• Etching acid can be applied well by using a small cotton sphere, a


small piece of foam, or with a small brush.
3. Imprinting

Another way is by using a fine syringe . As soon as all the


areas to be etched are acidic, the time is recorded, and the
email is etched for sixty seconds. If the acid used is in the
form of a solution, the liquid can be pressed on the surface
but must be careful because it can destroy the pores of
enamel that is being formed which is fragile.
4. Washing

• After sixty seconds the acid is washed clean. First of all


use a water spray from a triple syringe gnats most of the
acid is wasted. After spraying for five seconds, the air button
is also pressed so it will provide strong water and air spray for
15-20 seconds.

• If a gel form is used, the washing period must be doubled for at least 30
seconds to make sure that the gel and the product from the acid reaction
are clean. During the washing phase, the assistant takes the remaining
water with an aspirator.
5. Eating an e-mail

• The tooth surface is dried with air from a triple syringe . This phase
is very important because any moisture on the surface that has
been etched will prevent the penetration of resin into the enamel.

• The recommended drying time is at least 15 seconds. At this


stage the etched area must be clearly visible and white.
5. Drying the etched email

• It is always good to check whether the airways are not


polluted by water or oil. This can be seen by spraying on a
clean glass surface. The presence of moisture or oil from the
wind ducts will thwart the fissure closure.

• By using a rubber isolator, salivary contamination on the etched


surface does not have to occur. However, if this happens, it is
advisable to repeat the etching of the email, because saliva will close
the pores, which is very important for optimal bonding.
6. Mixing

• Ray resin material does not need to be mixed.

• Chemical resin (swapolimer), consisting of 2 components


which must be mixed slowly so that air bubbles do not
arise.

because incomplete hardening will cause failure. In addition
to molar teeth the source of light must be directed distally
from the occlusal surface for 60 seconds then to mesial for
60 seconds.
7. Application

• Every groove and pit in the buccal or palatal must receive the
same amount of irradiation and light directed directly above it.

• Most swapolymer resins harden in 1-3 minutes, and this can be seen
in the manufacturer's instructions for each ingredient. The outer layers
of each material will not hold polymerization because of the inhibitory
effect of oxygen in the atmosphere. Thus after poleminization this
cover resin will always appear oil coated.
8. Occlusal checking

Now the rubber isolator is removed and the occlusion is checked with

articulation paper.

If there is the use of a resin with a filler particle, it can be left with
the elevation of the teeth which are considered to be abarrated
themselves, then in the use of newer materials containing filler
particles it would be better if the elevated part was removed by
using a small diamond bur mounted on a conventional henpis
