In consideration of my
voluntary involvement in the intercollegiate athletic program(s) at the University of Memphis, I have had an opportunity to
carefully read the current CDC Notice and Travel Health Precautions regarding travel. I received educational information
provided by the University of Memphis Athletic Department discussing potential COVID-19 virus-related health risks and
recommended precautions. I was given an opportunity to ask medical staff and administration questions regarding the COVID-
19 virus during and after the meeting. The medical staff and administration answered my COVID-19 virus-related questions.

I acknowledge and agree to the following: (initial next to each for consent)

delays, increased security checks and additional air passenger restrictions. I have considered all of these risks, made my own
inquiry and investigation, and voluntarily agree to assume them.
____ I understand that symptoms may appear 2-14 days after exposure to the virus. Symptoms include: Fever or chills, Cough,
Shortness of breath or difficulty breathing, Fatigue, Muscle or body aches, Headache, New loss of taste or smell, Sore throat,
Congestion or runny nose, Nausea or vomiting, Diarrhea
____ I understand that if I feel should sick, I should complete the daily questionnaire and reach out to my assigned athletic
trainer immediately prior to reporting to the complex. At this point I will be notified to either stay at home, report to the athletic
complex or seek immediate medical care off-campus.
____ I understand that I must practice social distancing at all times. I will ensure there is 6-10 ft of space between myself and
other employees and students.
____ I understand that I must wash my hands and use hand sanitizer multiple times a day before and after touching areas of
high traffic (doorknobs, light switches, etc.)
I understand that face masks are required in all indoor and outdoor public spaces.
____ I understand that I should avoid social group gatherings on or off campus in hopes of preventing the spread of the virus
and protecting the integrity of each team and our athletic facilities.
____ I understand that I must complete my health assessment questionnaire prior to coming to campus or I will not be allowed
to participate in official workouts, practice, athletic training room appointments, etc.
_____I understand that there are temperature check stations outside of each athletic facility (Main Entrance Door).
_____I voluntarily and willingly choose to participate in returning to the University of Memphis campus.
_____ I voluntarily and willingly choose to participate in intercollegiate athletics on the University of Memphis campus.
_____ I voluntarily and willingly choose to participate in any travel associated with participation in intercollegiate athletics at
the University of Memphis.
_____ I further agree to voluntarily assume all risks related to the COVID-19 virus.
_____I also agree to comply with the Center for Disease Control and Prevention guidelines for travel as well as any notices
issued by the U.S. Department of State, http://www.state.gov/travelers/

RELEASE OF LIABILITY FOR ILLNESS (COVID-19)

The University of Memphis (“UofM”) Athletics Department has been approved to return to athletic related activities
(examples include, but are not limited to, strength and conditioning sessions, practices and associated practice activities)
for student-athletes. As you are likely aware, we are in the midst of the COVID-19 pandemic. COVID-19 is reported to
be extremely contagious. The state of medical knowledge is evolving, but the virus is believed to spread from person-to-
person contact and/or by contact with contaminated surfaces and objects, and even possibly in the air. People reportedly
can be infected and show no symptoms and therefore spread the disease. In addition, there are certain populations that
may be more vulnerable to COVID-19, including, but not limited to the elderly and those with underlying/pre-existing
medical conditions. The exact methods of spread and contraction are unknown, and there is no known treatment, cure, or
vaccine for COVID-19. Evidence has shown that COVID-19 can cause serious and potentially life threatening illness and
even death.

The UofM and its Athletics Department is dedicated to putting measures in place to help protect the health and safety of
student-athletes, coaches and staff. As a result, the UofM Athletics Department has developed guidelines and safety
measures related to limiting exposure and transmission of illnesses including COVID-19. Any student-athlete
participating in athletic related activities will be required to comply with all guidelines put forth by the University, it’s
athletic department, and the student-athlete’s specific sports team.

While the UofM Athletics Department has implemented its guidelines and safety measures with the health and safety of
its students and staff in mind, the UofM cannot prevent you from becoming exposed to, contracting, or spreading
COVID-19 while utilizing UofM property or engaging in athletic related activities. Therefore, if you choose to utilize
the UofM’s property and/or engage in athletic related activities, you may be exposing yourself to and/or increasing your
risk of contracting or spreading COVID-19.

ASSUMPTION OF RISK: I have read and understood the above warning concerning COVID-19. I hereby choose
to accept all risks associated with returning to campus and participating in athletics related activities, including, but not
limited to, risks associated with contracting COVID-19 in consideration for being allowed to participate in athletic related
activities at the UofM and/or enter the UofM’s property. Despite the risks, I voluntarily and willingly choose to return to
the UofM campus and participate in intercollegiate athletics on the UofM campus or other designated venues, including
but not limited to strength & conditioning sessions, practice session, games, team travel.

WAIVER OF LAWSUIT/LIABILITY: I hereby forever release and waive my right to bring suit against the UofM,
its Athletic Department, its employees, trustees, officers and agents from and against all claims, liability, rights, causes of
action, costs, attorney’s fees and expenses of any nature whatsoever, whether known or unknown, for any injury, loss, or
damage in connection with exposure, infection, and/or spread of COVID-19 related to my use of UofM property
and/or participating in athletic related activities. I understand that this waiver means I give up my right to bring any
claims including for personal injuries, death, disease or property losses, or any other loss, including but not limited to
claims of negligence and give up any claim I may have to seek damages, whether known or unknown, foreseen or
unforeseen.

I HAVE CAREFULLY READ AND FULLY UNDERSTAND ALL PROVISIONS OF THIS RELEASE, AND
FREELY AND KNOWINGLY ASSUME THE RISK AND WAIVE MY RIGHTS CONCERNING LIABILITY
AS DESCRIBED ABOVE:
