be other family members enrolled in a post-secondary institution for the 2010-11 academic
year.

It is the policy of the Financial Aid & Scholarship Office to review this information, as
individuals may ultimately change enrollment status, or enroll at an institution other than
the one previously indicated. To facilitate this process, we ask you to complete a form for each
family member attending a college other than UCSC.

Please follow these instructions for the form on the second page
of this letter:

• Complete the top of the form.

• Forward the form to each sibling enrolled in another college or university.
Your sibling(s) must complete and sign .
• Your sibling(s) should then forward the form to the Registrar at his/her
institution to complete .

Your financial aid may be reduced if you do not follow these

instructions.

The completed form(s) must be returned to the Financial Aid & Scholarship Office.
