NARRATIVE

Narrative:
On 061920 at approximately 0408Hrs., FUO was dispatched to the intersection of Lacrange Rd. and
Burlington Ave. for a report of an armed robbery. The following occurred in summary:

Upon arrival to the intersection, FUO was informed by dispatch that the victim, Jesus R. Maldonado, had
his residence in Hillside. R/O returned to this PD to contact the victim via telephone at

Maldonado related to R/O that at approximately 0350Hrs., he was traveling N/B on Lacrange Rd. and was
stopped at the intersection of Lacrange Rd. and Burlington Ave. waiting for the traffic light to turn green.

described as a M/B approximately 19-21YOA with a than build and wearing dark clothing appeared at the
drive/s side of Maldonado's vehicle. Maldonado stated at this time he thought the subiect to be a M/B by
the sound of his voice and his manner of speech. This subiect then demanded Maldonado cell phone (an

Maldonado reported that he turned his head slightly towards the subject and observed an unknown, dark

in color obiect in the subject's right hand.
Maldonado stated that he believed the object to be a black in color handgun, though did not see the
enough of the object-to confirm this. Maldonado then handed the subject his iPhone 11, noting that the
subjects ungloved left hand took the phone. This is when Maldonado noted the subjects dark skin
complexion. The subrect then demanded further to Maldonado "give me your pass code". Maldonado
then provided the pass code to unlock the iPhone to the subject. The subject then returned to the vehicle
behind Maldonado, though he was unsure which side ofthe vehicle the subject entered into. The vehicle
then changed lanes to the curb lane of N/B Lacrange Rd. and made a right turn onto E/B Burlington Ave.
towards Ogden Ave.

At this time, Maldonado observed the vehicle to be a crossover style sports utility vehicle, silver/grey in
color, possibly a Buick, unk license plate. Maldonado stated that he stayed at the intersection for a
moment due to being stunned and distressed from this event. Maldonado then retumed to his residence
in Hillside to contact this PD to report this event.

of the stolen iPhone. Sgt. Rohlicek related that the T-Mobilercustomer service person 'wendy' reported
that they refused to ping the phones' location due to that it "was not a life or death situation". Further
R/O notes that attempts to locate the phone using the iPhone Find my Phone App were unsuccessful.
Maldonado reports that, should a suspect be identified in this case, he is willing to sign a criminal
complaint.

On 061920 at approximately 0408Hrs., Ofc Niewold was dispatched to the intersection of Lacrange Rd.
and Burlington Ave. for a report of an armed robbery. Upon arrival to the intersection, Ofc Niewold was
informed by dispatch that the victim, Jesus R. Maldonado, had returned to his residence in Hillside. Ofc
Niewold returned to this PD to contact the victim via telephone ln Summary Maldonado

Maldonado's vehicle. Maldonado stated he thought the subject to be a M/B by the sound of his voice and
his manner of speech. This subject then demanded Maldonado's cell phone. Maldonado reported that
subject stated to him, "Give me your phone! Give me your fucking phone! l'm gonna light you up!". The
phone was provided to the offender with the offender fleelng the scqle E/B Burlington Ave from

further. R/O was assigned the investigation on tF-scase and theloflow-'ng was learned/occurred in
summary:

R/O went to Maldonado's residence in Hillside and observed the vehicle he was reportedly driving was
observed parked in the driveway of the residence. Utilizing a Department issued cell phone, FUO
obtained a picture of the vehicle which has been attached to the case file. The picture was taken while
R/O was driving on a public street past the residence. R/O noted a distinct beige stripe down the side of
the vehicle. A green color was noted above and below the stripe. The vehicle is an older model pick-up
truck namely a 2006 Chevrolet Silverado extended cab (small extended cab door behind the vehicle's
driver's seat).

This Department has access to and monitors video surveillance cameras located throughout the Village
at key points of security. R/O noted three of the cameras cover the area or are close to the area in
question. Based upon the times provided by Maldonado and his contacting this Department to file an
Armed Robbery Report, R/O reviewed two cameras which cover the intersection of Lacrange Rd and
Burlington Ave. The cameras are labeled as Lacrange Metra Street 1 and LaGrange Metra - Lacrange
Crossing. R/O noted both camera views are a distance from the intersection but does provide coverage
of the intersection. RyO reviewed from 0330hrs until 0407hrs when the report was filed and failed to
locate Maldonado's vehicle. Due to the distance of the camera and quality, R/O reviewed the video
footage from the Parking Deck - VH Fountain Camera for the same time in question. This camera covers
an area on Lacrange Road near Harris Ave. R/O noted that if Maldonado left his place of employment in
Mc Cook, Maldonado most likely would have driven past the camera in order to get to Lacrange Rd and
Burlington Ave which is one block south. The camera view includes all four lanes of traffic on LaGrange
Rd. R/O again failed to locate Maldonado's vehicle on the footage.

On 061920 at 1257hrs, FUO attempted to contact Maldonado by telephone on his wife's cell phone
Illeff a rnessage which isthe number provided by Maldonadoto ofc Niewold in order to make contact. R/o
for Maldonado to return the phone call. A few minutes later a subject called RIO'S phone
and identified himself as Jesus Maldonado. The following was related in summary:

go to McDonald's for a shake and fries. Upon remembering that his wife had been after him to stop
eating late at night and start eating healthy, Maldonado turned around in the Enterprise Rental Car lot
located just south of 55th Street on Lacrange Rd, Maldonado then proceeded N/B LaGrange Rd to
Burlington where the incident took place. Maldonado noted all his vehicle windows were lowered at the
time of the incident with the air conditioning at full force. After being robbed of his cell phone,
Maldonado drove N/B LaGrange Rd to Frontage Rd along 290 in Hillside. Maldonado drove west towards
his rGidence and turned on a couple of different streets but does not know their name. Once at the
residence, Maldonado sat in his car. Maldonado stated from the point of the robbery until arriving at
home it took approximately 20 mins. He further stated that he believed the robbery to have taken place
between 0000hrs and 0030hrs on 061920.

R/O noted there would have been a several hour delay in reporting the robbery. Maldonado advised he
did not have a phone to make the call to this Department. Maldonado also stated he was fearful the
offendeE would come to his residence. After sitting in the car for several hou6, Maldonado stated he
went into the residence and was attempting to get his wife's cell phone from under her pillow when she
woke up. Maldonado advised his wife of the armed robbery and called Hillside Police Deparknent's non-
emergency number to file a report. Upon learning the .eport would have to be filed with this Department,
Maldonado called 911 and was transfened to this Department.

R/O asked Maldonado if he had been drinking which he denied. Maldonado made an unsolicited
statement that he did not fall asleep in his vehicle. When asked further about the time he left work,
Maldonado stated he would be able to tell R/O the exact time due to a Workday App on his phone
however the phone had been stolen. Maldonado stated he was headed straight home with no additional
stops. After the robbery, Maldonado did not see a police officer otherwise he would have reported the
incident immediately. When asked why he stayed outside his residence, Maldonado stated there had
been a previous incident when a str?nger came to his house and it vvas very upsetting to his wife. Not
wanting to startle his wife, Maldonado stayed in his vehicle and lost track of time. lt was noted there
were several charges from Maldonado's phone but rtlaldonado was unable to provide exact information.
Maldonado stated he had no further infomation he could provide R/O in order to assist in this
lnvestigation.

R/O contacted Mike O'Connor the Supervisor for the Lyons Township Area Communications Center
(LTACC) which handles 911 calls and dispatches all calls for the Village of Lacrange. O'Connor was able
to provide R/O with a DVD-R (exhibit #1) containing the 911 call from Maldonado. The call was a 911
transfer from DuCom. ln the call, R/O noted Maldonado to be answering questions which were asked of
him. Maldonado stated the offender walked up to him as well as saying he pulled up to him. Maldonado
did state the incident had occurred 20 minutes prior to the 9'11 call. Maldonado also stated that he had
thought he had taken a picture of the offending vehicle's license plate but he did not have it. lt should be
not6d that Maldonado previously indicated he had only seen the offending vehicle as it was fleeing the
scene E/B Burlington Ave. When the vehicle was fleeing, the offender would have already had his cell
phone. R/O noted this Department was notified of the incident by Maldonado at O.O7hrs, Based upon
the information provided in the call, the incident would have occurred at approtmately O347hrs since the

(exhibit ,B) for documentation purposes and ease of review.

unable to say if Maldonado clocked out using the app which is allowed or by time clock within the
business. also stated he would be able to find out where Maldonado clocked out -from- but it
would have to go throuqh the corporate office in Houston. As of this report, has not provided the
information-to R/O. ,id provide R/O a copy of Maldonado's time sheet which has been attached to
the case file. -

Based upon the noted discrepancy in the time of occurrence provided by Maldonado, (between 0000h6
and 0030h6 as well as approximately 0345hrs) R/O conducted a review of video surveillance footage
from the Lacrange Village Hall Fountain Camera. The camera provides a view of all four lanes (2 - N/8,
which is closest to the camera, and 2 S/B) Lacrange Rd. The footage was reviewed from 1150hrs on
061820 to 0410hrs on 061920. R/O was unable to locate Itilaldonado's vehicle on the footage. The footage
was later transferred to a USB drive (exhibit #3) for documentation purposes and ease of viewing.

R/O contacted Det Lazansky from the Riverside Police Department. Det Lazansky has access to Vigilant
Solutions which documents vehicle images, license plates and locations of vehicles through Commercial
License Plate Reades. The vehicle was documented 18 different times from 't 20718 through 051420. R/O
noted that of the contacts, the vehicle was observed '15 different times between 2350hrs and 0600hrs. All
the night time contacts were in Chicago during the week. The printouts ofthe Vehicle Detection Reports
have been attached to the case file.

R/O spoke with Det Digangi of the Countryside Police Department and Lt Huedepohl of the Mc Cook
Police Departments. Sased upon Maldonado's stated route of travel it was determined thai there are no
public cameras which would cover the route of travel. Oet DiGangi also advised based upon his
knowledge of the video surveillance system at Enterprise Rental in Countryside, Maldonado would not
have been on came.a at the business due to a lack of exterior cameras.

A check of google maps indicated that there is a 14 minute travel time from Lacrange Rd and Burlington
Ave to Maldonado's residence in Hillside. lt was also noted that due to lnterstate 290, there are two main
streets which Maldonado would use to go north and south between Hillside and Mc Cook based upon the
location of Maldonado's house and a lack of north and south crossings over 1290. The streets were
Lacrange Rd and Wolf Rd. R/O noted Maldonado's vehicle was not on Lacrange Rd Cameras available
in the Village of Lacrange. Based upon prior knowledge, R/O knows Hillside to maintain a red light
camera at Wolf Rd and Roosevelt Rd.

R/O contacted the Hillside Police Department and spoke with Tony Millazzo (TX ). Millazzo
handles all inquires in reference to red light cameras. Millazzo stated the -rr- camera in question is
operational and covens N/B Wolf Rd. A continuous video would have to be reviewed as the Department
did not have a yiolation noted for Maldonado's yehicle. Millazzo provided R/O with a DVDr (exhibit #2)
containing the video surveillance from the red light camera. The files were later transferred to exhibit #3

for documentation purposes and ease of viewing. R/O reviewed the video provided (0000hrs on 061920
through (X1ohrs on 061920). R/O did not observe Maldonado's vehicle or a similar vehicle during the
recording. lt should be noted that the camera provides a view of the whole inteEection and traffic in all
directions can be observed and identilied.

R/O contacted Maldonado and requEted that he come to this Dopattment in rcference to this incident.
Maldonado agreed to come to this Department on 062320 at'1200h6 and bring documentation regarding
fraudulent charges made from his phone. l{aldonado arrived at 'l145hrs and was escorted to the
conference room adiacent to the lobby (incident ,n020-5957). lnv Kang and R/O intewiewed Maldonado.
Maldonado was advised he was present as the victim and was not under arrest. R/O stated the door to
th6 lobby was unlocked and noted Maldonado was seated closest to the door with no one preventing his
departure. The following was learned/occurred in summary:

Maldonado stated he had not been truthful when previously speaking with oftlcers. Maldonado stated he
wanted to tell R/O what had actually happened. On 061820 Maldonado started his shift at- .
Maldonado is usually early for the start of his shift and sits in his vehicle. After working his entire shift,
Maldonado clocked out at the time clock in the in Maldonado walked to his
vehicle and 16ft the facility. He did not go anywhere with his co-worke6. Maldonado instead drove
towards Chicago. When confronted with the license -{acility

hits in Chicago, Maldonado confirmed
that he routinely drives the areas on the west side and southwest sides of Ghicago. R/O not6d each area
is known for criminal activity. Maldonado denied using drugs or going to those areas for drinking. R/O
asked Maldonado if he would frequent those areas in order to find women willing to perform sexual acts
for money and Maldonado advised he does look for prostitutes,

On 061920 after leaving work from his shift that ended just before midnight on 061820, Maldonado stated
he went to the city and was looking for a prostitute. He said he has approached several prostitutes over
the last several months but has not made a deal. After driving around the city at locations he could not
recall, Maldonado stated he made a deal for a prostitute using the web site r-dilaldonado
responded to an ad by a prostitute and agr€ed to meet her at her residence. Maldonado believes the
address might be r- 61 Chicago. Maldonado had put the address into the GPS on his
phone and went to the aiea. Upon arrival, Maldonado did not feel right. He stated he has had a couple of
incidents involving previous deals where he has received photos of his house and the names of his
children while the purported prostitute tries to blackmail him for money. Due to the pr€vious problems,
Maldonado drove arcund -inthe area to check it out first. Maldonado believed a vehicle parked behind the
residence belonged to whomever he was supposed to meet so he took a picture using his iPhone.

Maldonado parked in front of the residence and sent a mGsage to his "date". The "date" appeared at the
front door but Maldonado noted the FrB was at a different building. The "date" messaged Maldonado
and stated the correct addrGs was.Zbut Maldonado was not certain without his phono as the whole
arrangement was made via text message. Maldonado walked up to the r€sidence and arround to a side

everywhere. Once inside the door, an unknown M/B approached Maldonado demanding his phone and
money while pointing what Maldonado believed to be a gun at him. Maldonado provided the $80.00USC
he had, the phone and his pass code. Maldonado left the building, drove directly home and went inside.
Maldonado stated he was never in Lacrange on the date and time in qu6tion. Maldonado stated he said

he was in Lacrange because when he took his wife's phone she woke up. Maldonado stated he panicked
because he did not want to tell his wife about the prostitute. Due to his wife knowing his normal route of
travel, Maldonado stated he was in LaGrange when the incident occurred. Since the incident, an
unknown offender has made two S2000.00USC withdrawals from Maldonado's Chase account via apple
pay on his phone. Maldonado was unable to provide documentation of the withdrawals stating he is
locked out of all his accounts. lt was also learned that Maldonado receives cash payments via cash app
and other payment app's. Maldonado uses the app's for his landscaping business.

R/O requested Maldonado sign a written statement in the matter in order to document his exact
statement. Maldonado kept stating he did not know what to do. RyO advised the statement was being
requested as part of a criminal investigation and that R/O could not advise him about what to do with
signing a statement. Maldonado again stated he did not know what to do. Ri O advised that a written
statement would be documented at his request since he did not know what to do and R/O would be
unable to tell him. lt was also advised that further investigation into the matter would be conducted in
order to determine if charges would be brought in connection with the incident. Maldonado was advised
to contact Chicago PD in order to file an armed robbery report in reference to the robbery and transfer of
funds. Maldonado left this Department at approximately 1330hrs.
At approximately l600hrs on 062320, RiO received a response from the Chicago Police Department
lntelligence Center (CPIC). R/O had requested information from CPIC on Maldonado and his vehicle. R/O
was advised that CPIC had conducted a check of Maldonado's registration through the municipal license
plate readers within the City of Chicago. The check of CPIC records indicated there were nine times
when Maldonado's plate ( | registered on their system. The locations and date/time are as

R/O noted the last four LPR hits were during times in which Maldonado stated he was in Lacrange to
R/O. lt was during these times that Maldonado stated he was the victim of an Armed Robbery in the
Village of LaGrange. Based upon a review of the case and evidence obtained during the investigation,
R/O determined probable cause existed for the arrest of Maldonado on the charge of Disorderly Conduct -
Filing a False Police Report. R/O contacted Maldonado and requested that he come to this Department.
Maldonado agreed to come to this Department on 062420 at 0800hrs.

On 062420 at 0713hrs, Maldonado arrived at this Department and was taken into custody by Ofc
Moncivais and Ofc Rogers. Maldonado was read his rights according to Miranda and did not make any
statements regarding this case post-Miranda.

R/O contacted the Cook County State's Attorney's Office Felony Review Unit. After hearing the facts of
this case ASA Robert Cannatello gave felony approval for one count of Disorderly Conduct filing a false
police report. Maldonado was processed and immediately transferred to Bridgeview Courthouse for the
purpose of a bond hearing.

R/O packaged all evidence in this matter according to Department Policy and Procedure. All exhibits
were entered into the BEAST evidence management system and placed in evidence locker #7.

The Armed Robbery occurring in Lacrange was unfounded with Maldonado being charged with filing a
false report.

Case closed.
