1 INTRODUCTION & INTENDED PURPOSE

This Emergency Response Plan (ERP) is designed to provide information on the response to
emergencies, which could occur on the NAWARA gas development project and the arrangements in
place to support the personnel in the camp/field if an incident should occur.
This ERP is going along with COPANY ERP.
It is the policy of CONTRACTOR to ensure, as far as is practicable, the health, safety and welfare at
work of all employees, and other persons who may be affected by CONTRACTOR’s activities and the
environment protection. To comply with this policy, CONTRACTOR will:

 Endeavour to safeguard personnel and property;


 Endeavour to supply a safe working environment at all times;
 Observe, as a minimum, current statutory obligations;
 Make provision for safety training;
 Encourage participation in all matters related to safety and accident prevention;
 Make available to all personnel sufficient information to enable them to undertake their duties in a
safe and efficient manner;
 Endeavour to limit incident consequences;
 Use goal setting to underpin and support our safety review; these requirements would be in the
areas of:
- Fire and explosion hazard management;
- Emergency response;
- Management and administration;
- Facilities (design and construction).
 Operate in such a manner to protect everyone in the field from the effects of fire and explosion
and to ensure that the responses to all potential emergencies are effective.
 Prepare emergency response plans which will provide sufficient information, to those who could
be involved, in the:
- Organization and arrangements, which come into effect in an emergency;
- Emergency response procedures to be followed in various circumstances.

1.1 Use of This Manual

CONTRACTOR has prepared this Emergency Response Plan to effectively and quickly respond to any
incident, which could affect the persons, the environment, or the field. The Emergency Response Plan
describes the main elements, which the Emergency Response Organization must implement and
maintain to effectively respond to any incident.

The core covers the incident scenarios. The majority of scenarios have been developed in this manual.
Each scenario includes a flowchart, which provides guidance on the actions to be taken within a certain
time.

On this plan, “Actions Plans” are provided on the opposite page to each Incident Scenario. These
Actions Plans have already been completed based on known information. They will be improved during
Emergency Response Drills and Exercises. The entire document is subject to ongoing improvement
and will be regularly reviewed and updated following exercises and potential emergencies.

The Head Office Emergency Management Team in Tunis consists of the CONTRACTOR Operations
Team, a Relatives Response Coordinator, and a Media Contact.
Logistics support is located at the shore base in Gabes.
All personnel, who have duties according to the Emergency Response Plan, will participate in plan
review meetings to become familiar with their responsibilities. Each response team member must be
thoroughly familiar and understands his specific role within the response team.

1.3 Definitions

The following definitions have been utilized thoroughly this document:

TERM DEFINITION
COMPANY (ETAP & 0MV) Is the party which carries out the detailed engineering, Site management,
supplies materials, equipment and services to perform the duties as specified
by the CONTRACTOR / OWNER;
COMPANY Representative means the member and/or members appointed or designated by COMPANY
in charge for the Double joint supervision on site as its duly authorized
representative to deal with CONTRACTOR with respect to the CONTRACT
Contact Directory A list of team members and key players in a crisis. The list should include
home phone numbers, cell phone numbers, e-mail address, etc.
CONTRACT is an agreement having a lawful object entered into voluntarily by two or more
parties, each of whom intends to create one or more legal obligations
between them
CONTRACTOR Entity (and its legal successors in title) named as CONTRACTOR in the
CONTRACTOR purchase order and/or the Contract awarded to the
CONTRACTOR to perform specific services and/or works
Crisis Is an actual or potential threat to the CONTRACTOR’s long-term ability to do
business due to the impact on reputation, legal / financial liabilities and the
ability to operate.
Emergency Is a situation from an incident, which has already taken place, but has the
potential to escalate further and cause additional damage to human life,
environment, assets, investment and reputation of CONTRACTOR
Incident Is a physical act that threatens human life, environment or property.
Job Safety Analysis (JSA) Risk assessment tools used to identify and control workplace hazards, which
helps, integrate accepted safety and health principles and practices into a
particular task or job operation...
PROJECT NAWARA Pipeline Project
Rescue Is the process of recovering of persons following their evacuation or escape
from the installation, and rescuing of persons near the installation and taking
such persons to a place of safety
Risk assessment Is the determination of quantitative or qualitative value of risk related to a
concrete situation and a recognized threat (also called hazard). Quantitative
risk assessment requires calculations of two components of risk the
magnitude of the potential loss (L), and the probability (P) that the loss will
occur.
SUBCONTRACTOR Entity (and its legal successors in title) named as SUBCONTRACTOR in the
CONTRACTOR purchase order and/or the CONTRACT awarded to the
SUBCONTRACTOR to perform specific services and/or works.
Work Method Statement A document that defines the methods to be used for the execution of a certain
(WMS) activity;

3.1 Crisis Management Team (CMT).

The CMT may be activated in response to a crisis which has come from an emergency, or it may
convene for an issue which has no underlying event. Its focus is on strategic issues which affect
continuing the PROJECT, profitability and reputation. The CMT may be led by the CEO of the
CONTARACTOR if the situation requires full leadership intervention, or control may be passed to
Project Manager to strategically manage a major emergency.

3.2 Emergency Management Team (EMT)

The Emergency Management Team (EMT) provides the support required by the field Emergency
Response Team (ERT) to facilitate its response to an emergency at site affecting the field involving the
NAWARA GDP crew, Gabes base, Ksar Ghilene, KP 158, KP 45 and Tunis Office Support.

The Tunis Office Emergency Management Team consists of an Operations Team, a Relative
Response Coordinator, and a Media Contact. This team is based in CONTRACTOR's head Office.
The following defines the key interfaces between these various groups.

Responsibilities:

 Establish and maintain communication with the emergency site;


 Initiate emergency alert and notification procedures as required;
 Contact and communicate facts about the emergency to government agencies;
 Monitor, direct and coordinate all phases of emergency response activities;
 Ensure that all appropriate emergency responses are carried out;
 Mobilize additional equipment, services & manpower as required;
 Direct inquiries and questions from media representatives to the Media Contact;
 Direct inquiries and questions from relatives and friends of personnel working In the field to the
Relatives Response Coordinator;
 Ensure that COMPANY is provided with current information;
 Initiate and maintain a time and event log.
The Emergency Management Team consists of the:

They continually review the onsite remedial information in order to assess the potential of the incident to

3.3 Emergency Control Center

The Contractor’s Emergency Control Center (ECC) is based in the meeting room, 5 rd flore of Lake
Forum Bloc B (5 Rue de la Feuille d’érable , 1053 Les Berges du lac 2).
CONTRACTOR's emergency activities will be coordinated from two locations:

 The Field activities will be directed by the Site Manager.

 The Emergency Control Center will be activated at any Tier of alert.

3.4 Field Emergency Response Team

3.4.1 Site Manager

The Site Manager is supported by other members of the team, who have been assigned specific duties
in the event of an emergency situation involving the field facilities. In case of emergency, the Site
Manager:

Initiates actions as per the Emergency Response Plan

Requests medical evacuation if required;


1. Completes preliminary incident/accident report.
2. Contacts EMT;
 During office hours via office direct lines/switchboard or mobile telephone;
 After office hours via mobile telephone and/or home telephone;
3. Prepares emergency message;
4. Informs Regional safety services of Tataouine
5. Requests neighborhood assistance if required;
6. Activates firefighting and rescue equipment, as required;
7. Activates safety equipment, as required;
8. Activates field evacuation procedure, as required.
Note: - The Site Manager or his official delegate is the only individual who has the authority to
activate the field evacuation procedure.

Note – The Site Manager will muster in the Emergency Response Center and assess the
emergency situation and keep the ECC of CONTRACTOR informed.

 Response to any emergency in or near the field will initially be by assigned personnel who
will assist the Site Manager throughout the emergency;
 The personnel will function in teams and will have at their disposal:

○ Comprehensive emergency response equipment for dealing with all foreseeable


events;
○ Emergency control and communication centers;
○ Emergency power for provision of crucial services, lighting and communications;
○ Adequate Emergency Response Plan.

3.4.2 Field Emergency Response Team

To assist the Site Manager in dealing with an emergency or the above interfaces, a number of

key personnel have been assigned specific responsibilities, notably:

3.4.3 Incident Response Team (IRT)

The IRT’s focus is on saving life, protecting property and minimizing damage to the environment. Each
facility, site or office has an IRT or identified first aid responders, capable of dealing with any emergency
or hazard that may be foreseen as a function of its operations. The IRT will normally consist of wardens
and first aid responders, with the main incident response effort provided by local Emergency Services in
accordance with the ERT instructions.
The IRT is the first responder to the emergencies with the following position as key persons:
 HSE site supervisor
 Construction Supervisor
 Field Medic
 First Aiders
 Camp Boss

4 LEVELS OF ALERT

4.1 Tier ONE Alert

A Tier one alert is declared, if an emergency occurs when:


 There is no immediate threat to the personnel health and safety;
 Personnel injuries if any are likely to be minor;
 Property or environmental risk is minimal;
 The situation can be handled entirely initially by Field personnel with minimum assistance from
CONTRACTOR without external assistance;

 A Tier ONE alert is declared as a precautionary measure.

4.2 Tier Two Alert

A Tier two alert is declared if:


 Personnel are involved in a serious injury or at risk of serious injury but not considered life
threatening;

 Emergency may cause damage to the environment or property;

 The incident probably extends beyond the control of the onsite Emergency Response Team and
would require support of the EMT and external agencies;

 EMT is fully mobilized;

4.3 Tier THREE Alert

A Tier three alert is declared when:


 Personnel are involved in a serious injury or at risk of injury which may be life threatening;
 Incident is a serious threat to personnel safety;
 Incident seriously threatens the environment or property;

 The incident extends beyond the control of the onsite Emergency Response Team, requiring
assistance of the EMT, and external and governmental agencies;

 EMT is fully mobilized.

5 EMERGENCY COMMUNICATION PLAN

5.1 General

5.2 Hazard Identification / Assessment

The process of hazard identification and assessment involves a thorough review that should include, but not
limited to:

 Transportation, materials handling, hoisting, equipment or product installation, temporary structures,


material storage, start-up, and commissioning activities
 Environmental concerns
 Consultation with the COMPANY regarding potential hazards when working in or adjacent to operating
facilities

 Resources such as Material Safety Data Sheets (MSDSs) to determine potential hazards from on-site
materials
 Proximity to traffic and public ways.

For each potential hazard it is important to identify resources necessary for an appropriate emergency
response. For most events in construction, a simple analysis based on the experience of the people involved on
the project is likely sufficient.

5.3 Emergency Resources

It is important to identify which resources are available to make up for any deficiencies.

It is essential to verify that Emergency contact numbers are in effect in the working site.

Other on-site resources such as fire extinguishers, spills containment equipment during fuel equipment refill,
and CONTRACTOR refueling points, First aid kits must be maintained and clearly identified. Construction
equipment may be included among potential emergency resources.

Personnel, especially on-site medical staff or trained workers in first aid, should be included in the plan.

Resources may have to be obtained and kept on site. ERP should include fire protection or ambulance / medical
resources in remote areas.

Whatever the situation may be, people, equipment, facilities, and materials are needed for emergency
response. Where they will come from must be determined in advance. Moreover, the people supplying these
resources must be made aware of their role in the plan.

5.4 Communication Systems

An important key to effective emergency response is a communications system that can relay accurate
information quickly. To do this, reliable communications equipment must be used, procedures developed, and
personnel trained. It is a good idea to have a backup system in place, in case the system is rendered useless.
For example, telephone lines may be cut.

The type and location of emergency communication systems must be posted on the project's site. This will
include a list of site personnel with cellular phones or two-way radios, and any other equipment available.
Emergency phone numbers and the site address/location should be posted.

A communication system must be made up of strategically placed equipment and properly defined
responsibilities. The ERP posted in a conspicuous place (i.e.: offices, CONTRACTOR camp sites, etc.) on the
project must identify the designated equipment and the people to operate it.

5.5 Administration of the Plan

The task of administrating and organizing the plan is vital to its effectiveness. The person who has this task will
normally be the person in charge of the emergency response operation. It is their task to ensure:

 That everyone clearly understands their roles and responsibilities within the ERP
 That emergency resources, whether people or equipment, are kept at adequate levels in step with the
progress of the project.

It is very important to review the ERP on a regular basis and especially after an emergency has occurred.

Changes may be necessary where deficiencies became apparent as the plan went into operation.

5.6 Emergency Response Procedure

An emergency can be reported from any source, a worker on site, an outside agency, or the public. The
circumstances may change during the course of an emergency. Any procedures that will be developed must be
able to respond to the ongoing situation.

The following list covers basic actions to be taken in an emergency. These steps apply to almost any
emergency and should be followed in sequence.

 Stay calm.
 Assess the situation.
 Take initiative.
 Provide protection.
 Aid and manage.
 Maintain contacts.
 Help emergency services.

5.7 Communication of the Procedure

To be effective, an Emergency Response Procedure must be clearly communicated to all site personnel. The
following activities have been considered:

 Review the procedure with new site subcontractors and new workers to ensure that it covers their
activities adequately.
 Review the procedure with suppliers to ensure that it covers any hazards that the storage or delivery of
their materials might create.
 Review new work areas in operating plants with owner/COMPANY to ensure that new hazards are
identified and covered in the procedure.
 Review the procedure with the Joint Health and Safety Committee or Health and Safety Representative
on a regular basis to address new hazards or significant changes in site conditions.
 Post the procedure in a conspicuous location.

The Emergency Response Procedure for this PROJECT must continually undergo review and revision to meet
changing conditions.

6 EMERGENCY PREPAREDNESS

Emergency preparedness is essential for effective emergency response. Essential elements


include:

• Chain of Command;
• Resources;
• Training;
• Public Education (Community Planning).

The organizational structure outlined above in section 3 identifies the EMT; the role of CONTRACTOR's
management in classifying the incident as a Level 1, 2 or 3; and how members of the ERT will coordinate
with and advise the management and Incident Commander.

The First Responder acts as the On-Site Commander until a member of the ERT, Incident Commander,
and/or a member of COOMPANY management arrives and gives direction to respond to the situation. If
the event is determined to be an emergency, the First Responder may call the Site Manager or his
delegate.

The Incident Commander is usually the on-site emergency responder. Until the Incident Commander
arrives, members of the ERT are responsible for securing the area, mobilizing the emergency response
personnel, accounting for all personnel and members of the public, overseeing public and environmental
protection, establishing and maintaining communications; and taking direction from the Incident
Commander.

Any incidents which require an emergency response will be post-appraised and documented by the First
Responder in conjunction with the CONTRACTOR ERT.

6.2 Resources

In preparing for emergency response, resource considerations should include standard medical kits, maps,
and drawings of the facilities. During operation of the project , each surface facility (pump station or
valve system) shall have current and correct maps, plans, directions and diagrams depicting key
safety elements and equipment located at each facility. Equipment operating manuals and shut-down
procedures will be prominently displayed at key locations at each facility.

Resources that include environmental clean-up response contractors, testing laboratories, licensed
disposal facilities, etc., will be included in the ERP once operations commence.

6.3 Training

Each sub-contractor and inspection firm working on the construction of the pipeline project will be
required to be trained by CONTRACTOR in health and safety issues prior to coming onto the job site. In
addition, daily safety meetings will be conducted both during construction and pipeline operation that will
inform employees of the emergency response procedures, location of safe refuge areas, directions to
medical facilities, emergency action plans, and the location of written documentation.
Prior to construction, sub-contractor foremen shall attend specific safety training that addresses
familiarization with the terrain and environmental issues in the project area. The agenda for this
specialized training may include:

• Seasonal considerations, such as weather: cold and hot;


• Geographical considerations: elevation and terrain;
• Particularly sensitive habitats;
• Travel considerations
• Clothing recommendations;
• Wildlife: Bugs to Snakes;
• Laws;
• Spill prevention;

During construction, field personnel will also be trained by CONTRACTOR in a variety of safety modules
to make the job site safe including:

 When and how to notify all other contractors when actions or activities undertaken by them could
affect health or safety of employees of other companies; to inform controlling contractor of all injuries
to workers; and who/how to report to controlling contractor any unsafe conditions that come to their
attention.

 Any used Chemical on site has to be justified by it's MSDS which must be available and clearly identified.
• Personal protective equipment is expected to be worn that may include protective
eyewear, gloves, hard hat, and footwear appropriate for the job site. Steel-toed footwear may be required
on a project-specific basis.
• Assessing whether additional safety precautions (e.g., grounding straps for vehicles
working near power lines) should be implemented as appropriate.

NOTE: Fire or Explosion


A fire or explosion might develop from an initially insignificant incident, which escalates out of control. Extreme caution is
always required during the response to such an emergency, and the paramount objectives for all personnel are the
protection of life, environment and property.

Generally, fires and explosions fall into two main categories:


Those which are quickly controlled, either by automatic systems or by the rapid intervention of Emergency Response Teams
(ERT) ;
Those, which rapidly escalate to a situation where an installation, evacuation, or abandonment is required.

In most cases, the Office response will be centered on the back-up support required to deal with the consequences of the fire
or explosion.

8.2 Oil Spill

8.2.1 Oil Spill Response Plan

The aim of this plan is to minimize the effect of any oil spill on the environment. For this purpose it
covers principally:

1. Alert flow and procedures of internal communication and with authorities and another relevant
parties;
2. Strategy, procedure and guidelines to be followed and action to be taken for oil spill response
and control. Any response is to include:
1.Stopping the leak
2.Containing the spill
3.Cleaning up the spill
3. Inventory of personnel and antipollution, and logistics available means and resources.

When reporting oil spills please observe the following:

A – Any oil spill, regardless of the volume, shall be reported without delay in accordance with

procedure.

B – Categorize spills in:

1. Type of spill: oil, condensate, diesel, other.


2. Volume spilled: in barrels or liters.
3. Size of spill and conditions: Weather condition
4. Location/coordinates: Field/base

The relevant authorities COMPANY, ME, ANPE, should be informed.

8.2.2 Oil Spill / Flammable Gas Leak

An oil spill/gas leak may be caused by the escalation of another incident or it may be an isolated
incident. In either case, notifications must still be made even though some organizations may
already have been notified of the emergency.

The steps in this procedure are additional to those that will be taken to control any emergency.

Any spill of oil or leakage of flammable gas (normally hydrocarbons) may cause an explosion and
fire risk on the facility, and can also result in hazards to:

-The personnel - gas burns, physical trauma, or gas asphyxiation, either due to gas liberating from
spilled oil, or due to a buildup of gas within confined spaces ;

The response to such an emergency commences with actions to reduce the possibility of a
fire/explosion, to ensure that personnel are withdrawn from hazardous areas, to stop the spill/leak if
possible and warn nearby facilities. The further response is essentially directed towards minimizing
the impact of an oil spill (including gas liquids) on the environment.

Note:
A – Toxic Gas Leak:
Toxic gas, not necessarily associated with hydrocarbons, may be encountered during production operations or as a by-
product of decomposition in water pits, water storage tanks, sewage storage tanks, inert gas, etc.
Warning: low concentrations of H2S can endanger the lives of personnel working on the field
Hydrogen Sulfide (H2S) is extremely toxic, corrosive and highly soluble in water and in oil. It is explosive in air range of 4.3%
to 45% by volume and has an auto-ignition temperature of 292oC.
Human tolerance of exposure varies greatly at low concentrations. As concentrations increase, tolerance variations decrease
rapidly. A feature of H2S poisoning is that the H2S knocks out that part of the central nervous systems, which controls the
body’s automatic breathing system.
Carbon monoxide (CO) is another toxic gas, which may be encountered in the exhausts of internal combustion engines or in
any space where incomplete combustion has taken place. Carbon monoxide is not as toxic as H2S but is still dangerous in
concentrations as low as 500ppm.

B- Flammable Gas Leak:


Warning: even during an emergency response, no-one should be allowed to enter an atmosphere which is immediately
threatening to life. No-one should be allowed to enter an oxygen deficient atmosphere without breathing apparatus.
The major hazard associated with non-toxic gas leaks is the creation of a flammable or asphyxiating atmosphere.
 Flammable Atmospheres
The major component of natural gas or associated gas is methane. Methane has a lower flammable limit (lfl) of 5% by
volume in air and an upper flammable limit (ufl) of 15% by volume in air; (below 5% the mixture is too lean to burn; above 15%
the mixture is too rich to burn).

Gas detection systems usually have monitors mounted in groups which are set to ensure that:
- An alarm is activated if any one head at 20% lel alarm only ;
- Any 2 heads simultaneously at 20% lel stop fans, start fwd deluge pump & starter fire pumps
- Any one head at 20% lel and any one head at 60% shut flaps and stop generation.

 Oxygen Deficient (Asphyxiating) Atmospheres


The normal oxygen content of air is 20.8% by volume. An oxygen deficient atmosphere is any atmosphere, which has an
oxygen content of less than 20.8%. The effect of oxygen deficient atmospheres on the human body depends upon the level
of oxygen, individual fitness and the type of activity undertaken.

NOTE: Failure

A failure of any item of equipment which could affect the safety of the project or the POB (Personnel on Board)
may, in certain cases, lead to a partial or total evacuation of the camp. The response to this category of emergency
will depend on the nature of the equipment failure and the possible consequences. This procedure is therefore
restricted and should be modified to suit prevailing circumstances. Actions to rectify the failure are clearly a part of
routine maintenance activities, but a response is required for the intervening period to ensure that safety is not
jeopardized whilst the equipment is out of service.

The speed of response to a lost man emergency is crucial to the person’s chances of survival. Weather conditions,
temperature, the victims clothing, weather conditions all affect the chances of a successful rescue. Once found, the victim

Infectious’ means that the disease may be passed on by airborne or other means. Contagious means that the
disease can be passed on by touch.
If the victim is to be returned to a city facility for further treatment, the transportation may involve Medivac or
Medrescue and could conceivably be handled immediately by the CONSTRUCTION MANAGER, Doctor and

Note: Bomb Threat / Terrorist Incident

A bomb threat is any threat of violence, which is aimed at any owned, operated or contracted facility.
A suspicious object is anything, which by some physical feature, or peculiarity of location, gives an impression of
abnormality which cannot be explained without investigation.
Any notification of a suspect bomb threat or act of terrorism shall be treated with the utmost seriousness and
confidentiality.
The advice and support of GN (Garde National / Military) will be sought on all occasions and their advice will be
followed.
The main priority in any bomb threat or terrorist incident is to comply with the immediate wishes of any persons
who are armed or threatening staff. Thereafter attention must be given to safeguarding life and reporting details
to the GN as soon as possible. Safeguarding life is most likely to involve temporary withdrawal of all personnel
working on the field to a safe location. In the more predictable forms of threat or incident this withdrawal to a safe
location will center on the accommodation area, and if bombs have allegedly been place at unknown locations,
the area will require searching prior to the withdrawal.
All forms of bomb threat or terrorist incident are to be conveyed to the Emergency Desk/EMT in the first instance.
Communications should be kept as confidential as the conditions permit.

8.8 Injury Investigation and Work Resumption

In every case of an emergency involving a fatality, serious injury, and loss or damage to company
property, a company representative should proceed immediately to the field for investigation.

Particular care shall be exercised to ensure all evidence is preserved in its original state.

8.9 Accidents with a Fatality or Serious Injury

Following an accident where fatality or serious injury has occurred, governmental agency
representatives may decide to carry out an investigation into either the extent or cause of the
situation.
After presenting their credentials, they are to be afforded full cooperation in the performance of their
duties.
Work at the scene of a fatality may not be resumed until permission has been obtained from the GN
OuedZar / Tataouine. However, resumption of work is always permitted to facilitate rescue
operations or when failure to resume operations may endanger the lives of others.

8.10 Accidents with Loss or Damage to Property / Facility

Where loss or damage to company property or production has occurred, evidence should not be
distributed until permission has been received from the Emergency Desk.

9 CONTRACTOR REPORTS

9.1 Internal Reporting of Information

The following information must be reported:


 The time and location of emergency;
 The type of emergency - human life, environmental, fire, etc.;
 Personnel injuries;
 Cause of situation, out of control or controlled;
 Severity - estimate of amount of hydrocarbon released;
 Weather conditions
 Action being taken;
 Persons or agencies already notified.
In the event of an emergency, each person involved will immediately start a log of events and times of
all occurrences, noting names, times of contacting other people or agencies, reasons for various
actions, and summaries of statements given and to whom.

9.2 External Reporting of Information

If an occasion arises that outside reporting is required by regulations or may be a necessary for
assistance, remember – “DO NOT SPECULATE”.

Provide information under these general guidelines:


 The time and location of emergency;
 The type of emergency - quantities released, spilled. (Be conservative);
 Description of emergency. (Be brief, do not speculate, especially as to cause or fault.);
 The area affected;
 The corrective action being taken and plans for additional response.

9.3 Statutory Reports

Reports required by governmental regulations shall be promptly prepared, and where practical,
submitted for approval to the Project Manager and COMPANY Project Manager prior to forwarding
to the government agency. Statutory reports must be prepared with care, reporting only facts and
expressing no opinion as to cause.

9.4 Preliminary Reports

Preliminary Reports may be required in the event of death, serious injury, or possible litigation. If a
Preliminary Report is required, no other written report than statutory reports shall be prepared until
COMPANY provide alternate direction.

At the emergency site, the senior company employee shall have the responsibility of handling
accredited news media personnel and advising them that information on the accident can be
obtained from the COMPANY Project Manager. Although news media personnel have no more rights
than a private citizen to enter company property, they should be tactfully handled, always accompanied
while on company premises, and, for their safety, limit access to danger areas.
