university; that in case that I have a pending application for admission to any other higher
educational institution. I undertake to withdraw the same upon signing of this undertaking;

5. I will submit the hard copy of the requirements* on or before June 30, 2020 to:

6. I understand that I will be bounded to enrol at Laguna University once I gave my consent to the
Registrar’s Office to register me.

7. I understand that to be admitted to the Board Programs (BEED, BSED, BSA, BSME),
the following conditions are required:

• BSME, BSA, BEED, and BSED - a General Weighted Average (GWA) of 85; no grade below
80 in the Report Card;

• BEED and BSED - could be a graduate of any strand

• BSA - should be a graduate of ABM strand only

• BSME - should be a graduate of STEM strand only

• Original College Academic Records/Certificate of Grades signed by Registrar from previous
school for evaluation purposes.

• Original Transcript of Records with remarks “Copy for Laguna University” (to be submitted
before the end of the semester)
• Original Certificate of Good Moral Character signed by Guidance Counselor
• Photocopy of PSA authenticated Birth certificate
o For female married applicant, submit photocopy of PSA
authenticated Marriage Contract
• 2 pcs. Passport Size Picture with Name Tag
• Long brown envelope
