FACILITY FOR THE ELDERLY]

I am MS. MAYBELYN C. GALVEZ, an architecture student of ARCP05 from National University.

I am currently undertaking a thesis research study on “Assisted Living Community with Nursing
Facility for the Elderly” for our Architectural Design 9: Thesis Research Writing (ADESGN9) class,
under the supervision of Arch’t. Arianne Joy Q. Dullas.

Under Executive Order No. 2, s. 2016, Freedom of Information Order, I am writing to request your good
office for the following public information/records for my data gathering:

*Updated and latest Demographic of Elderly citizens in the Philippines

*Updated and latest Demographic of Homeless elderly citizens in the Philippines
*Updated copy of Common Health Issue of Senior Citizens
*Updated and latest copy of Existing Assisted living for the Elderly and or Home for the aged in the
country
*Latest and updated copy of Number of Poor Senior Citizens in the Philippines
*Latest and updated copy of Number of Abandoned Senior Citizens in the country
*Updated number of Senior Citizens who are Abused by their Family
*Updated number of Senior Citizens who are Abused by the Public
*Updated copy of Existing Nursing Facilities for Senior Citizens in the Philippines
*Top 10 places in the Philippines that has the Most Number of Senior Abused Cases

If possible, I would like to request a waiver of all fees, if any, for searching or copying the public
information/records mentioned above as these will be used for academic purposes only.

I would appreciate your advice if the information requested could not be disclosed nor released within the
timeframe prescribed by Department of Health and R.A. 11032, Ease of Doing Business and Efficient
Government Service Delivery Act of 2018.

Upon request, I will be happy to provide your office a copy of the full report by the end of the academic
