INCIDENT CASES

1. Unattended electric stove that was left open and caused fire in an apartment which
caused damage in the property.

How it should not happened/should had been prevented?


The people in the apartment are responsible for their appliances. They should
know how to use it properly. They should not have left the stove open and they should
always be minded that if they’re cooking, they should double-check if the stove is still
turned on.

2. Machine malfunctioning in an industrial plant due to rusting.

How it should not happened/should had been prevented?


Always secure the maintenance of machine to prevent or to at least keep it from
rusting in a short span of time.

3. Drinking liquor knowing that you have alcohol intolerance and caused your skin to
swell.

How it should not happened/should had been prevented?


The person should have taken his allergy medication before drinking alcohol in
order to prevent the allergy attack.

4. Forgetting security password on your bank account.

How it should not happened/should had been prevented?


If the person is aware that he easily forgets things, he should have saved the
password on his phone or told a trusted person.

5. Leaving the hospital without the final check-up of the physician and his approval that
you could be discharged already then being readmitted with the same case.

How it should not happened/should had been prevented?


The person should not be stubborn no matter how bad he wanted to be
discharged already. He should have waited for the doctor to avoid any more possible
injuries and readmittance.
ACCIDENT CASES
1. A child drowning in a pool.

How it should not happened/should had been prevented?


Young children can drown in very shallow water, so should be supervised at all
times when near it.

2. Family that has cold that resulted into medicine poisoning.

How it should not happened/should had been prevented?


It is important to look for the expiration date of the medicine being intake. It is
also important to throw away medicine that is already expired and restock new medicine
instead.

3. Injuries that are caused by inadequate Personal Protective Equipment (PPE).

How it should not happened/should had been prevented?


Ensure all PPE is maintained in good working order, have adequate storage
and that repairs, maintenance and replacements are carried out as needed. Also the
people that will be wearing it should be given proper guidelines on how to use it.

4. Chemical burns from contact with corrosive materials, acids or oils.

How it should not happened/should had been prevented?


Training for employees that work with dangerous chemical or combustible
materials is needed and they should be given protective clothing.

5. Cuts and lacerations from coming in contact with sharp tools or parts of machinery.

How it should not happened/should had been prevented?


Ensure all moving parts are stopped before attempting to move any
dangerous machinery and proper training for the workers.

COVID-19 HAD BECOME A PANDEMIC, CAN YOU STATE IF IT IS AN


ACCIDENT OR INCIDENT. JUSTIFY YOUR ANSWER.
Covid-19 being a pandemic is an incident. It is a situation that affected the whole
globe. The moment that it spread in Wuhan, China, they should have alerted all
the countries to close all flights coming in and out in order to prevent other
people from being infected. The way they handled things resulted into Covid-19
being a pandemic.
The person who may have eaten the bat didn’t know about the consequences thus
Covid-19’s existence is an accident. But for the virus to be a pandemic, it’s an incident
caused by improper hazard management.
