For NURSERY and Kindergarten:
1. PSA Birth Certificate (Original and Photocopy)
2. 2 pieces – 1 x 1 colored ID photo with white background
3. 2 pieces – 2 x 2 colored ID photo with white background
4. Doctor’s Certification (at the back of the Student’s Health Record Form)

For GRADE 1 to 12 and Tertiary/College:

1. PSA Birth Certificate (Original and Photocopy)
2. 2 pieces – 1 x 1 colored ID photo with white background
3. 2 pieces – 2 x 2 colored ID photo with white background
4. Doctor’s Certification (at the back of the Student’s Health Record Form)
5. ECCD Checklist for GRADE 1
6. Form 138 (Report Card for Grade 1 to 12)
7. Entrance Examination Result (both oral and written)
8. Good Moral Certification
