I hereby undertake to enrol for Summer Classes for the Academic Year 2019-2020 and I acknowledge the following
conditions of enrolment:

1. I am fully aware that this summer course will be using a purely online format, including the exams from June 8 to
July 31.
2. I will enroll in only one subject for the Summer Term.
3. I intend to enroll in an English course simultaneously with my chosen law course. I have a

deficiency in the (0) units required for English subjects.

4. I understand that priority will be given to potential 2020/2021 Bar Examinees for Labor Law

Review and Legal Forms. I have pre-enlisted. I acknowledge that I am not sure to have a slot.

5. I have a computer and access to the internet during the entire period of the Summer Classes.
6. I have access to library resources and I will be able to purchase textbooks (online purchase), if the

professor prescribes one.

7. I am aware that the Summer Term may coincide with the integration period or final exams of the 2nd

semester of AY 2019-2020.

8. My program is (please encircle): NTJD, NTJDP, JDCTR, JDCTRP.

9. I have (100) academic units (exclude English deficiency courses) to take before I graduate.
10. I will graduate and take the bar exams scheduled in: 2020 (in 1Q 2021); 2021; 2022; 2023.
