1. This is to announce that the Schools Division Office cf City of Malolos is now accepting applicants for
Teacher I positions for elementary, junior and senior high school for School Year 2017:2018-

2. Puisuant to DepEd Order No. 7, s. 2015 "Hiring Guidelines for Teacher I Positions effec{ive School
Year (SY) ?015-2016," the Division Selection Committee provides the following schedute af activities

 An applicant shall rcgisterto the Department's online syslem at wvw.applicatian.deped.qov.pf, where
they must encnde their Personal Data Sheet and select the Division where they want to be ranked.
Once submi'tted, an Applicant Number will be issued. ln the submission of apgication requirements,
this APPLICANT NUMBER MUST BE If{DICATED.

4. All applicants shall submit their application to the nearest elementary and secondary school of their
residence or school of their choice together with their letter of intent and pertinent dbcuments as listed
below and placed in two(2) long white folder with fastener with the Applicant Nurnber indicated.

b. Duly Accomplished Applicantb lndividual Sheet (pls. see attached Annex A) ischool
c. Omnibus Certification of authenticlty and veraoity of all documents submitted signed by the
applicant and notarized (please see attached Annex C)
d, CSC Fonn 212 (revised 2005) - Personal Data Sheet with latest passport size lD picture
e. Phatccopy ot PRC lD; or PRC Certification showing the teacher's name , LET rating, and
other information recorded in the PRG effice;
f. Photocopy of ratings obtained in the Licensure Examination for Teachers (LET)/Professional
Board Examination for Teachers (PBET)
g. Certified photocopy of Transcript of Records
a. Baccalaureate Degrees

5. ln case any one of the above-required documehts is lacking, the Schoolscreening Committee shall
not accept the application;

6. As stated under Section 4.4.4 o{ DepED Order No.7, s. 2fi15, teachers who have not prac-ticed their

7. All folderc and documents submitted by teacher-applicent shall be considered as recods of the
DMsion and shall no longer be retumed to the applicant.

8. Only those who obtained a total score of seventy (70) points and above will be included in the
Registry of Qualified Applicants {RQA).

9. The RQA shall be valid only for a period of one {1i School Year.

10. All school indurling fiscal autonomous schqols are required to subrnit their Schaol Selection
Committee following tlepEd Order No. 7, s. 2015 until January 19,2012.
