A. Title of Form:​ Change

B. Responses Tab:​ See
student responses.
C. Add-Ons:​ Include an
Add-On to change the
form to be more efficient.
D. Pallet (Theme): ​ Change
the color scheme or
include an image.
E. Preview Form:​ Look at
the live form.
F. Settings:​ Various options
for modifying the form.
G. Send Form:​ Send the
form either as email,
link, or HTML for website.
H. Options:​ Gives even more options for modifying the Form.
I. Add Questions:​ Provides various types of questions that can be added to the form.
J. Add Title:​ Add a title to the form, besides the one already provided.
K. Add Image:​ Add an image, which could be used for questions.
L. Add YouTube Video:​ Provides the video directly into the form.
M. Add Section:​ By adding sections, the form will be split into multiple pages that require
the user to click “Next.”
N. Copy Question:​ Provides a replicate of the question.
O. Delete Question:​ Deletes the questions from the form.
P. Required:​ Require the user to answer the question, or else they can not move on in
the form.
Q. Required Button:​ Repeat
R. Question Options:​ Different options are provided based upon the type of question.
S. Change Question Type:​ Various different options of questions to ask participants.
T. Form Description:​ Give info on what to expect from the form.
U. Answer Options:​ Varies based upon the type of question.
V. “Other” Option:​ Allows participants to provide their own answer as opposed to those
provided.

1. Short Answer:​ Great for 1-2 word
answers, including Name and
Password.
2. Paragraph:​ More essay style
question.
3. Multiple Choice:​ Traditional
multiple choice style question with
the option for “other.”
4. Checkbox:​ Allow participants to
“check” the answer(s) that best
apply to the question.
5. Dropdown:​ Similar to multiple
choice, but in a dropdown manner.
6. Linear Scale:​ Allow participants to select along a sliding scale of options set the by
the test designer.
7. Multiple Choice Grid:​ Great for doing matching style questions (Tip: Keeping it to 5
questions per grid works best for participants).
8. Date:​ Allow participants to select a date.
9. Time:​ Allow participants to select a time.

Tip: If selected, then students can “send
copy” to themselves.

Tip: Keep open in case student has an issue
while taking the test.

Confirmation Page:
1. Provide your own message for
participants when they complete the form.

Question Types (Options)

➢ Multiple Choice/Dropdown:

1. ​Description:​ Allows added


details for the question.
2. ​Go to section based on
answer:​ Allows participants
to go to a different section of
the form based upon which
answer they choose.
3. ​Shuffle Option Order:
Great for changing the
answer options to make
cheating more difficult.

➢ Multiple Choice Grid:

1. ​Limit to One Response Per


Column:​ Great for keeping
students from answering the
same option accidentally.
Tip: Do in conjunction with
Require one response per
row
2. ​Shuffle Row Order:​ Do it
help avoid cheating.
