Rules to be imposed at Checkpoint Areas

1. ALL PUBLIC TRANSPORTATION VEHICLES (Ceres Bus, Cebro, Van, Motorcycles) with PASSENGERS going to
Sagay City shall be instructed to STOP at CHEKCPOINT areas, and ALL OF ITS PASSENGERS will be considered
as PERSON UNDER MONITORING (PUM) and shall be instructed to DISEMBARK & WAIT IN THE HOLDING
AREA for PROFILING. While, those with NO PASSENGER(S) going to SAGAY CITY shall be allowed to pass
through and shall be escorted to the exit point by the PNP.

2. For PRIVATE VEHICLES (SUV, Motorbikes, Multicab, Sedan, etc.)

a. ONLY those who intend to go to SAGAY City shall be instructed to park and wait for PROFILING.
b. Those who do not intend to go to SAGAY City shall be allowed to PASS and shall be escorted to the
exit point by the PNP.

3. CONTAINER VANS/ TRUCKS carrying basic commodities and construction materials shall be allowed to PASS
through and deliver the goods to their destination. They shall be escorted by the PNP.

HOME QUARANTINE
A. Without private vehicle- wait for LGU
provided vehicle to transport PUM to
their BRGY. HALL and to be endorsed to
the BRGY. Capt/BHERT. (indi kinhanglan
updan sang CHO on duty ang PUM nga e
transport, e endorse lang sa vehicle
DRIVER kag siya na lang ang ma endorse
sa BRGY).
B. With Private Vehicle (to fetch or already
available) - may proceed to their
residence for home quarantine (instruct
PUM nga e inform nya dayun ang iya
BHERT/BHW na PUM siya kag for home
quarantine).
PROCESSING PROCEDURE
1. E-ask kung san-o ang LAST DAY naghalin ka sa AREA
nga MAY CONFIRMED COVID-19?

2. Add ka dayun 14 days = Katapusan Nga adlaw sang
home quarantine(mm/dd)____________
3. Fill-upan sang CHO ang KATAPUSAN NGA ADLAW
nga PARTE sang HOME QUARANTINE GUIDELIINES
BY THE DOH FORM.
4. E- explain sang CHO ang Guidelines & Contract.
5. Pa fill-upan kag pa PERMAHAN sa PUM ang DOH
form kag Contract.
6. Kuhaon sang CHO ang lower portion sang Contract
kag ihatag ang nabilin nga Pormas sa PUM. Ang
lower portion sang contract e endorse sa CHO

7. Dispatching process.
or LGU QUARANTINE
Wait at the holding area for the LGU vehicle,
LGU vehicle shall transport PUM to LGU
identified quarantine area (Temporarily:
Balay Kauswagan). Indi kinhanglan updan
sang CHO on duty ang PUM nga e transport, e
endorse lang sa vehicle DRIVER kag siya na
lang ang ma endorse sa DepEd Nurse-on-Duty
sa Kauswagan).
