Outcomes of an accident/incident

qNegative Aspects
vInjury and possible death
vDisease
vDamage to equipment and property
vDamage to the environment
vLitigation costs, citations
vLost productivity
vLow morale
Outcomes of an accident/incident
qPositive Aspects
vLessons can be learned
vChange in environmental program or
process
vChange in procedures
vChange in equipment design
vPrevent occurrence of more severe
accident/incident
Accident/Incident Investigation

vWill determine how and why of failures.

vExamine possible corrective action.
vAid in the accident/incident prevention
and elimination of a clearly identified
hazards.
vMost important-Investigation is not
intended to place blame except when
gross negligence could be established.
ACCIDENT/INCIDENT
INVESTIGATION
vA methodical effort to collect and interpret
the facts of accident/incident
vAn inquiry as to how and why the
accident/incident occurred in order to
explore actions that should be taken to
prevent or minimize recurrence of the
accident/incident.
PRINCIPLE OF ACCIDENT/INCIDENT
INVESTIGATION
The investigation of any accident or
incident event/s will never progress
unless that event is first properly
reported to the appropriate
management within an
organization
PRINCIPLE OF ACCIDENT/INCIDENT
INVESTIGATION

A Formal Policy requiring the

proper and consistent reporting of
all accidents/incident is one of the
most important principles of any
accident investigation program.
TYPES OF INCIDENTS TO BE
REPORTED
— Fatal accidents
— Accident causing injury or illness or
