Analyze each of the following case incidents, and make analyses and
recommendations. (10 points each)

1. A supervisor entered an injection molding machine while inspecting for


the source of a hydraulic leak when the machine cycled. The supervisor died
from massive head injuries. Two fixed guards were removed about 12 days
prior to the accident.

Answer: Possible cause of death dealt with massive head trauma due to lack
of PPE if there is an available PPE on the supervisor on the time of the said
incident serious head injuries could have been prevented. Thus saving the
life of the supervisor. Another possible reason to his/her death is lack of
communication. The fixed guards were already removed 12 days before the
incident happen and still no one advises or no one notice that something is
missing.

Possible recommendations are :

a. Wearing Proper PPE whenever conducting a machine checkup up

b. Always turns off the machine if we are going to check it closely

c. Perform weekly or daily (if possible) visual inspection and


maintenance on dangerous machines that are running non stop or for
a long period of time.

2. A set-up person was killed when he entered the machine to remove a


jammed part. The rear guard interlock had been inoperable for about 6
months.

Answer: Clearly the cause of his death is lack of machine maintenance and
negligence of the current situation of the machine. The set up person
doesn’t respect the fact that there is something wrong on the machine for
quite some time now and still he/she insist to remove something out of it
without taking any precautionary measures or proper repair on the
machine. In this kind of incidents, having PPE are not enough if we disregard
the facts and the nature of the machine we are handling.

Recommendations:

a. Proper information on the machine that we are dealing with can save
our lives. Knowing how it works and its history of troubles can save
our lives as technicians/engineers and operators.

b. Keep machine on tip top condition which means proper repair and
maintenance . some problem cant be solved by quick fix solutions
often this is the cause of major machine trouble and loss of lives in a
workstation.

3. An operator crawled under the machine to pick up parts while it was


operating.  His head was crushed when the platen opened.

Answer:

Possible cause of death is negligence to safety machine protocols and unsafe


decisions in the part of the operator due to lack of information and
knowledge about safety procedures while disregarding the fact that the
machine is still running when he does his act leading to the operators
demise.

Recommendations:

a. Turn off the machine before conducting any repair or doing anything
that requires close encounter to its dangerous processes or
components.

b. Proper safety training should be required to all employees especially


to those who are using the machines at first hand.
c. Having safety guidelines posted all around the work area, keeping the
employees reminded that there are risk on the workstation and what
are the possible cause of their actions,
