PROCEDURE TO BE ADOPTED WHEN A FATAL OR

SERIOUS INCIDENT OCCURS
POLICY

All Mine Employees are adopt these procedures in case of serious incident.

OBJECTIVE

To ensure that the correct procedures are carried out effectively when a fatal or serious
incident occurs.

GANGER OR SHIFT SUPERVISOR

1 . On receiving the message from the messenger, he will immediately proceed to the scene
of the incident / injury.

2 . Immediately send for the First Aid equipment, and render same in case of an injury.

3 . Assess the extent of the injury , and notify the nearest official.

4 . Notify the Shaft Controller giving details of the severity of the incident / injury eg. Head ,
back, legs, neck or hip injury, and requesting ambulance , rescue cars, medical service,
Proto teams and other services.

SHAFT CONTROLLER

The Shaft Controller must record all particular and notify the Mine Overseer and Section
Manager .

SCENE OF THE INCIDENT

Rescue operation to commence immediately in the safest and quickest possible manner.

Where the scene of the incident has to be cleared in order to prevent further injuries or as
per Regulation 25.4 all relevant data such as position of locomotives, cars, bodies, have
to be clearly marked on the hanging , sidewall or footwall. The scene of the incident must
be barricaded off and not be tampered with, unless instructed otherwise by a senior official
(Mine Overseer, Manager etc)

CONTINUITY WITNESS
The injured person or the deceased must be accompanied to the Hospital or Mortuary by a
reliable witness – Crew Supervisors should be used from working place up to the shaft to
Hospital and Mortuary . As the Continuity witness is merely responsible for accompanying
the body from the scene of the incident to the mine hospital , and thence to the Government
Mortuary , any underground worker may be used to perform this function.

RECORD OF INCIDENT

The Shift Supervisor or responsible person will record all the names and numbers of the
witnesses , and as much as possible concerning the incident in his note book. He should also
make a sketch plan with all relevant distances pertaining to the incident or estimate where it
is impossible to measure these safely.

This information must be passed on to the Mine Overseer concerned.

HUMAN RESOURCE AREA MANAGER

1 . He will arrange to have all witnesses associated with the incident to accompany the
Inspector of Mines for inspection at the scene of the incident.

2 . He will arrange for an eye witness and a witness who is a relative or a close friend of
the deceased to identify the body at the Mortuary.

3 . He will ensure that the witnesses who are to proceed to the mortuary are handed over to
the H.R.Area Manager on the day prior to the identification of the deceased.

4 . He will complete forms Hostel 19 (7 copies) and Inspector of Mines Enquiry (7 copies)
and pass onto the Section Manager two days prior to the enquiry.

5 . He will ensure that all witnesses are present at the enquiry.

6 . Provide an Interpeter.

MEDICAL SUPERINTENDENT

1 . In the case of a serious injury , he will immediately proceed to the injured and assume
responsibility for all medical treatment.

2 . In the case of a fatality , he will certify the person dead.

3 . Ensure that the correct witnesses proceed to the mortuary for identification of the deceased.

4 . Should there be a discrepancy , he will report the matter to the Personnel Officer.

SECTION MANAGER

1 . Report the incident to the Inspector of Mines as soon as possible.

2 . Notify the Business Area Manager or Business Manager and Safety Officer.

3 . Arrange for a Survey to accompany the Inspector of Mines to the scene of the incident,
for the purpose of taking measurements.

4 . Arrange to have a plan and section of the scene of the fatality prepared on a scale specified
by the Inspector of Mines . Eight copies to be available at the enquiry.

5 . Notify all relevant employees concerning the date , time and venue of the inquiry.

MINE OVERSEER

1 . Pass on all data concerning the deceased to the Safety Officer (background of deceased).

2 . Pass on all numbers of the employees required to attend the enquiry to the
H.R. Area Manager and Safety Officer.

SAFETY OFFICER

1 . Proceed to scene of incident with the investigation team as soon as possible.

2 . Take photos of the scene of the incident, check on witnesses and cause of incident.

SAFETY OFFICER

3 . Investigate incident with investigation team.

4 . Assist in completing all relevant documentation.

5 . Submit documentation and photos to Business Manager.

6 . Advise the Group Safety Consultant of the fatal injury after permission
has been granted by the Business Manager.

7 . Advise the Chamber of Mines Statistical Department of fatal as soon as possible

by telephone, and confirm same in writing.

8 . Supply information to Hospital Superintendent . Form MD 16 to be completed (5 copies).

9 . Accompany the Inspector of Mines to the scene of the incident.

10 . Check on documentation and witnesses before the enquiry.

11 . Attend the in - loco enquiry.

12 . Attend in house enquiry.
