DISCHARGE PLAN

PATIENT’S OUTCOME CRITERIA NURSING ORDER

MEDICATION  Instruct the patient to comply with the ordered home medication
prescribed by the physician
 Instruct the patient to take medications on the right time and to
complete her medications as prescribed by the physician.
 Advise patient to immediately report any side effects

EXERCISE  Inform patient and SO the importance, purpose and benefits of


passive ROM exercise.
 Teach the client on how to do proper deep breathing by inhaling
deeply and exhaling deeply. Encourage the patient to ambulate
but should not stress her out much.
 Instruct patient's SO to do ADL's gradually
 Instruct patient to have adequate rest and sleep at least 8
hour/day to avoid exhaustion

TREATMENT  Observe for any pain, Instruct SO to to maintain home


environment that is stress free, which could promote rest and
relaxation to patient

HEALTH TEACHING  Educate the patient and SO the importance of adequate rest
periods to prevent fatigue Educate them the importance of the
intake of nutritional foods
 Discuss the importance of daily hygiene

OUTPATIENT  Instruct patient to immediately seek consult when the following is


developed; uncontrolled and unmanaged pain, infection.
 Remind patient to adhere to doctor's appointment post discharges

DIET  Instruct patient to continue the prescribed diet


 Eat healthy foods such as fruits, vegetables, and meat. Drink a lot
of water, at least 8-10 glasses of water a day.

SPIRITUAL  Give sense of encouragement that God should be the source of


hope and she should thank him by praying. To strengthen his
relationship with God
 Encourage patient to do guided imagery such as healing imagery,
pain control imagery and relaxation imagery, music therapy
