Hi everyone! I hope you are all safe and healthy.

Before I give you the instructions for your assessments, I want to make sure that you know that you are
only to do the assessments if you are able to. These cover the finals period of our semester and are
designed to be doable offline.

I understand that there are also tasks given to you for your other subjects that may be overwhelming and
not to mention the conditions brought about by the pandemic that may have cause a great amount of
stress on your part. I want to assure you that I will be available if you need help or further guide in
understanding the materials and accomplishing these tasks. I will also post other supplementary materials
that may help you in further understanding the topics given.

Moreover, please remember that even if you are not able to complete the activities by May 18, you will
only be marked as IP but you can still submit until the given extension of the University. Try doing the
activities one at a time and at your own pacing.

IV. Formative Assessment #8 (Synthesis)

Type: Individual

Instructions

For this activity, you would have to channel your own ways and best practices in dealing with stress. The
title of this activity is “My Self-Care Kit”.

 Think of the different facets in your life that you encounter stress but choose at least
three contexts that you want to include only. It can be your life in school, social life, at
home, in cyberspace, even in the society, and the like.
 Then, think of the things that you do when you find yourself in stressful situations on
those settings. Select two or more of your best practices or research ways (that you can
apply personally) upon dealing stress for each facet of your life.
 Insert pictures/images that represent those practices in your chart (see image below for
reference).

 Then, on a separate page, write a short discussion or explanation of your self-care or self-
management kit. On this part, you may insert your sources and own anecdotes on how these ways
help you get through stressful experiences.
