1 CONTRACT DETAILS

2 SCOPE OF WORKS

The structural scope of work will be:

- Installing formwork of first floor slab

3 SEQUENCE OF WORKS

4 Site Preparation:

a. Before commencing the work, the area shall be clean isolate from other work.
Material to be used shall be kept nearby in one place.
b. Toolbox Meeting shall be implemented for all personnel who involve in this scope
work(scaffolder).
c. Any related work permit such as permit to working at height shall be approved
before commencing the work.
d. Safe access to for working area shall be maintain and well preparation
e. Temporary fall protection on working area shall be constructed to prevent any
falling hazard.
f. Trained operator or flagman will be engaged to control or operate heavy
equipment.
g. Ensure the source of supply to ready mix and grade of concrete is confirmed.

5 Step 1:
a. Before installing beam formwork, rebar reinforcement and casting concrete
suspended slabs, we have to erect scaffolding structure in order to support the
bottom of floor and beam.
b. Before starting the work. Survey or site in charge supervisor shall be survey the
area to locate the base point which will stand on the jack base.
c. After that we will deploy scaffold on each jack base.
d. We have two option of scaffolding which erect for support beam and scaffolding
for support concrete floor slab, For scaffolding support beam we will erect in 3
layer of scaffolding, First layer (type:1.7m), Second layer (type:1.2m), and third
layer (type: 0.9m). for scaffolding support slab we will erect in 3 layer of

scaffolding. First layer (type:1.7m), second layer (type:1.2m), and third layer
(type:1.2m).
e. Each layer of scaffold will be equipped with X-bracing for chaining from a scaffold
to another scaffold. Horizontal tubular bracing will be applied also for
straightening the position of entire scaffolding. Horizontal bracing shall be
equipped from 2 meters from bottom and continue up until reach below roof slab.
f. While we are erecting the scaffolding. Safe access for worker working shall be
applying simultaneously during erect scaffolding. Steel ladder and platform will be
in place for area of erecting scaffolding.
g. Once the scaffolding erected on top layer. We will installing the U-head jack on
hole top of scaffold. U-head jack could be squeeze up or down for support the
steel joist on top.
h. The gap of U-head jack from one to another shall be 0.6m to 0.7m.
i. Once U-head jack is installed and joist was in place. We will deploy formwork of
roof floor slab.

6 Step2:
a. Once base of scaffold is completed and secure, we will lift bottom formwork of
beam to plot on its marked position by crane, Signal man and lifting supervisor
will be at place for this operational.
b. Once bottom formwork are secure and on right position, beam rebar
reinforcement will be lift by crane and plot on between both secure column
accordingly.
c. After that we will install both of beam side one by one with secure welded support
steel tube (50x50@1.6mm) and welding between bottom formwork and beam
side formwork steel tube (50x100@1.6mm) will be weld and attached on head
tree and align with beam formwork which will be able for strut to be stand on for
building (welding between beam formwork and below steel tube).

7 Step 3:
a. Once scaffold is secured and stable with horizontal and vertical bracing, we will plot
steel tube (50mm x 100mm) as stringer attached on U-head jack. Top level of
stringer shall be same level of top inner of floor beam. Shoring and props shall be
added to support stringer.
b. After that we will lay steel mesh (6mm x 200) above stringer. Note: location and
cover steel mesh shall be followed to construction drawing of floor beam and slab.
c. Steel mesh shall be lift by crane to drops on floor position and lifting supervisor will
be supervising at place to secure the safety operational.

8 Step 4:

a. When scaffolding, beam formwork, are secure and follow construction drawing
design, we will cast the concrete.
b. Access way for concrete truck and pump truck have to be maintain and exclusion
from non-personnel.

9 Step 5:

a. After 16-24h concreted casted. Curing shall be take place in order to feeding the
concrete slab and straighten quality of casted concrete.
b. Beam formwork, scaffolding, props and shoring shall be remove out. Once the
concrete testing result is passed for 7days 14days, and 28day,

7 SAFETY, HEALTH & ENVIRONMENT

7.1 Information to personnel

a. Before job start to check if protective & safety requirements are provide.
b. Appropriate PPE (Personal Protective Equipment) must be used by personal
doing task.
c. Pre-job talk must be done all personal assigned to do the task must understand
the hazard associated with the job and the control measures in place
d. Safety induction must be given to all the personnel prior to entering the site and
adequate supervision by safety officer and site supervisor to ensure the job is
carried out in the safety way possible.
e. Non-essential personnel will be kept well away from the area of operation and
Bank-men will be provided where necessary.
f. Always wear harness for scaffolding during erection.

7.2 Use of PPE

a. All personnel will be issued with and instructed to always maintain safe working
practices and the civil engineer shall be responsible to ensure safety
procedures are followed.
b. Safety Helmets must be used by everyone, in area, where there is a potential is
of falling objects or when passing under congested pipe and equipment.
c. Personnel working in the vicinity of loading operation will wear safety helmets,
safety footwear and high-visibility vests.

7.3 Hand Tools and Equipment

a. All tool and equipment shall be in good working condition.
b. Operator shall be instructed in the safe use of the equipment by Camatec team
engineer/safety personnel prior to use.
